The role of the Dental Surgeon in the prevention of ventilator-associated pneumonia

Resumo: Ventilator-associated pneumonia is at risk of developing with the use of mechanical ventilation, which extends the patient's hospital stay by approximately 5-9 days, in addition to increasing hospital costs. Regarding the etiopathogenesis of VAP, it is known that it occurs through the airways that are contaminated by oral pathogens, by inhalation or aspiration of saliva that is contaminated by bacterial enzymes and oral bacteria. The aim of the present study was to explain the importance of the dentist in the prevention of ventilator-associated pneumonia in the hospital environment and the clinical procedures used. The study was carried out through a literature review, through searches on PubMed, Scielo and Google Scholar, between the years 2017 to 2022, contributing to the construction of this review. In the research carried out in the databases, the descriptors were used, among which 681 articles were found, among them 575 in Google Scholar, 105 in PubMed and 1 in Scielo, finally, only 20 articles were included in the literature review. Thus, it has been proposed that the presence of a dentist, related to the use of chlorhexidine, can reduce the incidence of VAP. Therefore, the dentist is increasingly necessary in the hospital environment with the multidisciplinary team, especially in ICUs due to their important role in the treatment and maintenance of health in hospitalized patients.

En cuanto a la etiopatogenia de la NAVM, se sabe que se presenta a través de las vías respiratorias que se encuentran contaminadas por patógenos orales, por inhalación o aspiración de saliva que se encuentra contaminada por enzimas bacterianas y bacterias orales. El objetivo del presente estudio fue explicar la importancia del odontólogo en la prevención de la neumonía asociada a ventilador en el ambiente hospitalario y los procedimientos clínicos utilizados. El estudio se realizó a través de una revisión de literatura, a través de búsquedas en PubMed, Scielo y Google Scholar, entre los años 2017 a 2022, contribuyendo a la construcción de esta revisión. En la investigación realizada en las bases de datos se utilizaron los descriptores, entre los cuales se encontraron 681 artículos, entre ellos 575 en Google Scholar, 105 en PubMed y 1 en Scielo, finalmente solo 20 artículos fueron incluidos en la revisión de literatura. Así, se ha propuesto que la presencia de un odontólogo, relacionada con el uso de clorhexidina, puede reducir la incidencia de NAV. Por ello, el odontólogo es cada vez más necesario en el ámbito hospitalario junto al equipo multidisciplinar, especialmente en las UCI por su importante papel en el tratamiento y mantenimiento de la salud de los pacientes hospitalizados.A pneumonia associada a ventilação mecânica tem o risco de se desenvolver com a utilização da ventilação mecânica, com isso estende o tempo de internação do paciente cerca de 5-9 dias, além de aumentar os custos hospitalares. Em relação a etiopatogenia da PAVM sabe-se que ocorre pelas vias aéreas que são contaminadas por patógenos bucais, pela inalação ou aspiração da saliva que se apresenta contaminada por enzimas bacterianas e bactérias orais. O objetivo do presente estudo foi explicar sobre a importância do cirurgião-dentista na prevenção da pneumonia associada a ventilação mecânica no ambiente hospitalar e as condutas clínicas usadas. O estudo foi realizado por meio de uma revisão de literatura, por meio de buscas no PubMed, Scielo e Google Acadêmico, entre os anos de 2017 a 2022 colaborando para construção desta revisão. Na pesquisa realizada nas bases de dados foram usados os descritores, dentre os quais permitiram encontrar 681 artigos, dentre deles 575 no Google Acadêmico, 105 no PubMed e 1 no Scielo, por fim, apenas 20 artigos foram incluídos na revisão de literatura. Destarte, tem sido proposto que a presença do cirurgião-dentista, relacionado com a utilização de clorexidina consegue diminuir a incidência da PAVM.
