Implementation of the neurological skills ambulatory in medical graduation: practices and reflections

Resumo: | Medical education in Brazil is heterogeneous and has undergone a reformulation in recent decades. Activities were resized and the role of specialty clinics needs to be frequently revisited. Neurological patients have become frequent in primary care and professional training needs to address this scenario. The study aims to describe the experience of implementing an academic neurology outpatient clinic, as well as the teaching-learning process developed in the Medicine course at a university in the interior of the Amazon, and to suggest solutions to the problems encountered. It is a descriptive study, in the form of an experience report, with a qualitative approach. Most students had difficulties with content related to neurological diseases and performing the neurological exam, with progressive evolution over the months. The diagnostic reasoning had a satisfactory level and also progressed. The most persistent problems were related to time management, description in the medical record, preparation of prescription and communication of diagnosis and prognosis. The assessment methods used in the outpatient clinic were self-assessment, immediate feedback, structured objective clinical examination (OSCE) and summative assessment through analysis of videos of real or simulated patients. The assessment was satisfactory for most. It was concluded that the neurology outpatient clinic is an important setting for the integration of teaching, care and the development of skills necessary for professional practice. |

Las actividades se redimensionaron y el papel de las clínicas especializadas debe revisarse con frecuencia. Los pacientes neurológicos se han vuelto frecuentes en la atención primaria y la formación de los profesionales necesita hacer frente a este escenario. El estudio tiene como objetivo describir la experiencia de implementación de un ambulatorio académico de neurología, así como el proceso de enseñanza-aprendizaje desarrollado en la carrera de Medicina en una universidad del interior de la Amazonía, y sugerir soluciones a los problemas encontrados. Se trata de un estudio descriptivo, en forma de relato de experiencia, con abordaje cualitativo. La mayoría de los estudiantes presentó dificultades con los contenidos relacionados con las enfermedades neurológicas y la realización del examen neurológico, con evolución progresiva a lo largo de los meses. El razonamiento diagnóstico tuvo un nivel satisfactorio y también progresó. Los problemas más persistentes estaban relacionados con la gestión del tiempo, la descripción en la historia clínica, la elaboración de la prescripción y la comunicación del diagnóstico y pronóstico. Los métodos de evaluación utilizados en la consulta externa fueron la autoevaluación, la retroalimentación inmediata, el examen clínico objetivo estructurado (ECOE) y la evaluación sumativa mediante el análisis de videos de pacientes reales o si006Dulados. La evaluación fue satisfactoria para la mayoría. Pacientes neurológicos tornaram-se frequentes na atenção primária e a formação profissional precisa contemplar este cenário. O estudo objetiva descrever a experiência de implantação de um ambulatório acadêmico de neurologia, bem como o processo de ensino-aprendizagem desenvolvido no curso de Medicina em uma universidade no interior da Amazônia, e sugerir soluções para os problemas encontrados. É um estudo descritivo, sob forma de relato de experiência, com abordagem qualitativa. A maioria dos alunos tinham dificuldades com conteúdos relativos a doenças neurológicas e execução do exame neurológico, havendo progressiva evolução ao longo dos meses. O raciocínio diagnóstico tinha nível satisfatório e também progrediu. Os problemas mais persistentes relacionavam-se ao gerenciamento do tempo, descrição no prontuário, elaboração de receita e comunicação de diagnóstico e prognóstico. Os métodos avaliativos usados no ambulatório foram autoavaliação, feedback imediato, exame clínico objetivo estruturado (OSCE) e avaliação somativa através de análise de vídeos de pacientes reais ou simulados. A avaliação foi satisfatória para a maioria.
