Endodontic treatment of premolars with three root canals: case series

Resumo: The present study aims to report a series of clinical cases of endodontic treatment of maxillary and mandibular premolar teeth with three root canals, with the aid of magnification and modern operative techniques of endodontic therapy.The endodontic treatment of these teeth was based on a careful radiographic evaluation and the access was performed with the use of magnification, which allowed the location of the holes in the root canals and the conduction of the endodontic treatment. The chemical-mechanical preparation was performed with mechanized nickel-titanium instruments (X1-Blue) associated with 2.5% sodium hypochlorite irrigating solution. As an auxiliary protocol in the final cleaning, a passive ultrasonic irrigation (PUI) was performed. At the end, the technique of filling with a single cone associated with the endodontic sealer was used. The presence of three root canals in premolars has a low prevalence, however, it is extremely important for the professional to know about the possible anatomical variations, since this can influence the success of endodontic treatment. Given the limitations, the correct management of these cases can be attributed to a good radiographic evaluation, the use of magnification and clinical criteria that, together, helped to obtain the best possible result.

El tratamiento endodóntico de estos dientes se basó en una cuidadosa evaluación radiográfica y el acceso se realizó con el uso de magnificación, lo que permitió la localización de los agujeros en los conductos radiculares y la realización del tratamiento endodóntico. La preparación químico-mecánica se realizó con instrumentos mecanizados de níquel-titanio (X1-Blue) asociados a solución irrigante de hipoclorito de sodio al 2,5%. Como protocolo auxiliar en la limpieza final se realizó una irrigación ultrasónica pasiva (PUI). Al final se utilizó la técnica de obturación con un solo cono asociado al cemento endodóntico EndoFill. La presencia de tres conductos radiculares en premolares tiene una baja prevalencia, sin embargo, es de suma importancia que el profesional conozca acerca de las posibles variaciones anatómicas, ya que esto puede influir en el éxito del tratamiento endodóntico. Dadas las limitaciones, el correcto manejo de estos casos se puede atribuir a una buena evaluación radiográfica, el uso de magnificación y criterio clínico que, en conjunto, ayudaron a obtener el mejor resultado posible.O presente estudo tem como objetivo relatar uma série de casos clínicos do tratamento endodôntico de dentes pré-molares superiores e inferiores apresentando três canais radiculares, com auxílio da magnificação e modernas técnicas operatórias da terapia endodôntica. O tratamento endodôntico desses dentes foi baseado na avaliação radiográfica criteriosa e o acesso realizado com uso da magnificação, o que permitiu a localização dos orifícios dos canais radiculares e a condução do tratamento endodôntico. O preparo químico-mecânico foi realizado com instrumentos de níquel-titânio mecanizados (X1-Blue) associados à solução irrigadora hipoclorito de sódio a 2,5%. Como protocolo auxiliar na limpeza final foi realizada uma irrigação ultrassônica passiva (PUI). Ao final empregou-se a técnica de obturação com cone único associado ao cimento endodôntico. A presença de três canais radiculares nos pré-molares tem uma prevalência baixa, no entanto é de suma importância o conhecimento do profissional acerca das possíveis variações anatômicas, visto que isso pode influenciar no sucesso do tratamento endodôntico.
