Resolution of severe facial asymmetry caused by osteochondroma through temporomandibular joint surgery associated with bimaxillary orthognathics

Resumo: Facial asymmetry can result from numerous types of diseases, such as: trauma (edema, emphysema, hematomas) and serious infectious processes that generate volumetric increases in the face quickly and noticeably. On the other hand, the development of cysts and tumors, changes in development/growth of both hard and soft tissues, are slower processes and consequently perceived when there is a significant change. Osteochondroma (OC), one of the intraosseous lesions can be defined as a protrusion of bone cartilage that arises from the outermost surface of the affected bone. The objective of this work is to describe a complex, but stable and satisfactory treatment methodology with good stability and satisfactory aesthetic-functional results. Discussion directly related to therapeutic modality, cost-effectiveness of localized treatment compared to combined treatment, risks, benefits and the like. The results show images that make it possible to verify the accuracy and applicability of virtual planning in the correction of deformities associated with the expertise of surgeons with great experience in surgical treatments, ending with a satisfactory result for the patient and team.

Por otro lado, el desarrollo de quistes y tumores, cambios en el desarrollo/crecimiento tanto de tejidos duros como blandos, son procesos más lentos y consecuentemente percibidos cuando hay un cambio significativo. El osteocondroma (OC), una de las lesiones intraóseas, se puede definir como una protuberancia del cartílago óseo que surge de la superficie más externa del hueso afectado. El objetivo de este trabajo es describir una metodología de tratamiento compleja, pero estable y satisfactoria con buena estabilidad y resultados estético-funcionales satisfactorios. Discusión directamente relacionada con la modalidad terapéutica, costo-efectividad del tratamiento localizado en comparación con el tratamiento combinado, riesgos, beneficios y similares. Los resultados muestran imágenes que permiten verificar la precisión y aplicabilidad de la planificación virtual en la corrección de deformidades asociadas a la pericia de cirujanos con gran experiencia en tratamientos quirúrgicos, finalizando con un resultado satisfactorio para el paciente y el equipo.A assimetria facial pode ser decorrente de inúmeros tipos de doenças, tais como: traumatismos (edema, enfisema, hematomas) e processos infecciosos graves que geram aumentos volumétricos em face de forma rápida e notória. Já o desenvolvimento de cistos e tumores, alterações de desenvolvimento/crescimento tanto de tecidos duros, quanto partes moles, são processos mais lentos e consequentemente percebidos quando há uma alteração significativa. O osteocondroma (OC), uma das lesões intraósseas pode ser definida como uma protusão de cartilagem óssea que surge a partir da superfície mais externa do osso afetado. O objetivo do trabalho é descrever uma metodologia de tratamento complexa, porém estável e satisfatória com boa estabilidade e resultados estéticos-funcionais satisfatórios. A discussão diretamente relacionada com modalidade terapêutica, custo-benefício do tratamento localizado comparado com o tratamento combinado, riscos, benefícios e afins.
