Control of oral biofilm and its relationship with the reduction of respiratory infections in ICU patients: a review of clinical trials

Resumo: This study aimed to evaluate, through a review of clinical trials, the importance of dental care and oral health care for patients in Intensive Care Units (ICUs), for the control of oral biofilm and respiratory infections. A search was carried out in the MEDLINE / PubMed and LILACS databases, using the descriptors “Dental Care”, “Hospital Dental Staff”, “Intensive Care Unit”, “Dentist, including only randomized clinical trials, published until July 2020, without restriction of language and country of study. The selected studies were submitted to initial screening based on the reading of titles, abstracts and full article, with duplicate studies being excluded. 43 articles were found, of which, only 10 randomized clinical trials were included in the present review. Considering the results found in the present review, the oral health of patients in ICUs, when neglected, can represent an important source of dissemination of respiratory infections, which highlights the importance of dental care and oral care in a hospital environment , with the use of 0.12% chlorhexidine, not associated with tooth brushing, to control oral biofilm and reduce the incidence of respiratory infections.

Se realizó una búsqueda en las bases de datos MEDLINE / PubMed y LILACS, utilizando los descriptores “Dental Care”, “Hospital Dental Staff”, “Intensive Care Unit”, “Dentist, incluyendo solo ensayos clínicos aleatorizados, publicados hasta julio de 2020, sin restricción de idioma y país de estudio. Los estudios seleccionados se sometieron a un cribado inicial basado en la lectura de títulos, resúmenes y artículos completos, excluyéndose los estudios duplicados. Se encontraron 43 artículos, de los cuales, solo se incluyeron 10 ensayos clínicos aleatorizados en la presente revisión. Considerando los resultados encontrados en la presente revisión, la salud bucal de los pacientes en UCI, cuando se descuida, puede representar una fuente importante de diseminación de infecciones respiratorias, lo que resalta la importancia del cuidado dental y el cuidado bucal en un ambiente hospitalario , con el uso de clorhexidina al 0,12%, no asociada al cepillado de dientes, para controlar el biofilm oral y reducir la incidencia de infecciones respiratorias.O presente estudo teve como objetivo avaliar, através de uma revisão de ensaios clínicos, a importância da assistência odontológica e dos cuidados com a saúde oral de pacientes em Unidades de Terapia Intensiva (UTIs), para o controle do biofilme oral e de infecções respiratórias. Realizou-se uma busca nas bases de dados MEDLINE/PubMed e LILACS, utilizando os descritores “Dental Care”, “Hospital Dental Staff”, “Intensive Care Unit”, “Dentist, incluindo apenas ensaios clínicos randomizados, publicados até julho de 2020, sem restrição de idioma e país de estudo. Os estudos selecionados foram submetidos à triagem inicial com base na leitura dos títulos, resumos e artigo completo, sendo excluídos os estudos em duplicata. Foram encontrados 43 artigos, desses, apenas 10 ensaios clínicos randomizados foram incluídos na presente revisão.
