Development of surgical guidelines for auxiliary technique of gingivectomy on inner bevel with osteotomy: report case

Resumo: The objective was to report a clinical case of gingival smile correction associated with passive eruption altered by means of gingivectomy in internal bevel and osteotomy with reverse planning and assisted by surgical guide developed to facilitate the technique, where did the questionnaire of OHIP and EVA to evaluate the improvement in quality of life and the pain in the postoperative period. A 23-year-old patient was dissatisfied with his smile, which had a high gingival exposure with short clinical crowns. In the clinical examination, an increase in bone volume was observed in the anterior region of the maxilla, thus diagnosing altered passive eruption. Before the surgical act was carried out the molding, to be done waxing from the data collected. After these steps, a surgical guide was developed, fenestrated with acrylic resin, following the planned measures, being able to guide the surgical incisions. After 21 days of the surgical procedure, the new measures were re-evaluated and compared with those stipulated in the preoperative planning, in which the increase in the clinical crown of the elements was 100% equal to the planned one, thus demonstrating the effectiveness of the guide surgical fenestration, improving the quality of life and providing minimal discomfort. Knowing that esthetics is closely linked to the quality of life of the individual, it is essential to use a predictable planning, using techniques such as diagnostic waxing and surgical guidance that provides a more reliable procedure.

Un paciente de 23 años no estaba satisfecho con su sonrisa, que mostraba una exposición elevada de las encías con coronas clínicas cortas. En el examen clínico, hubo un aumento en el volumen óseo en la región anterior del maxilar, diagnosticando así como una erupción pasiva alterada. Antes de la cirugía, se realizó la impresión, para ser encerada a partir de los datos recopilados. Después de estos pasos, se desarrolló una guía quirúrgica fenestrada con resina acrílica, siguiendo las medidas planificadas, para así poder guiar las incisiones quirúrgicas. Después de 21 días de la cirugía, las nuevas medidas fueron reevaluadas y comparadas con las estipuladas en la planificación preoperatoria, donde se encontró que el aumento en la corona clínica de los elementos era 100% igual al planificado, lo que demuestra la efectividad de la guía. procedimiento quirúrgico fenestrado, que mejora la calidad de vida y proporciona un mínimo de molestias. Paciente de 23 anos estava insatisfeito com o seu sorriso que apresentava uma exposição elevada de gengiva apresentando coroas clínicas curtas. No exame clínico observou-se um aumento de volume ósseo na região anterior de maxila, diagnosticando assim como erupção passiva alterada. Antes do ato cirúrgico foi realizada a moldagem, para ser feito o enceramento a partir dos dados coletas. Após essas etapas foi desenvolvido um guia cirúrgico fenestrado com resina acrílica, seguindo as medidas previstas, sendo assim, capaz de nortear as incisões cirúrgicas. Após 21 dias do ato cirúrgico foi feita a reavaliação das novas medidas e comparando-as com aquelas estipuladas no planejamento pré-operatório, onde constatou que o aumento da coroa clínica dos elementos foi 100% igual ao planejado demostrando assim a eficácia do guia cirúrgico fenestrado, melhorando a qualidade de vida e proporcionando mínimo desconforto.
