Low Intensity Laser photobiomodulation in the treatment of major aphthous stomatitis – two year follow-up

Resumo: Recurrent aphthous stomatitis (RAS) is one of the most common pathologies in routine dentistry, manifested through necrotizing ulcers that are, in most cases, quite painful and that can last for up to 14 days, affecting the patient's quality of life. Although uncertain, studies indicate that its appearance may be related to the immune system, lack of vitamins, genetic predisposition, among other factors. Its treatment consists of reducing the patient's symptoms and the duration of the lesion in the oral cavity, associated with drugs or therapies that speed up the repair and analgesia process. Low-intensity laser irradiation (LLL) has been used and studied as an auxiliary therapy for RAS. It has, among several pros, analgesic, anti-inflammatory and regenerative effects. The aim of this study was to analyze the effectiveness of LLLT in tissue repair, analgesia and recurrence of recurrent major aphthous lesion in a patient selected from the Hebiatrician at the University of Mogi das Cruzes Dental Clinic followed up since 2019, in which contact length irradiation was performed of an infrared wave of 808nm and a dose around 105J/cm2 in 4 intraoral points, followed by an extraoral application for lymphatic drainage, in two sessions with an interval of 24 hours, with follow-up after 60 days and after 2 years. Using the visual analogue scale (VAS), the patient indicated the intensity and evolution of pain, which was reduced from 10 (severe pain) in the 1st session to 1 (mild pain) in the 2nd session and remained at 0 in the 3rd session. after 60 days and after 2 years. Clinically, the lesion took around 14 days to heal and with irradiation the time decreased to 3 days, with no recurrence after 2 years. LLLT has been shown to be effective in tissue repair, analgesia and recurrence of major aphthous stomatitis lesion for two years, proving to be an effective and reliable treatment.

Aunque es incierto, los estudios indican que su aparición puede estar relacionada con el sistema inmunológico, falta de vitaminas, predisposición genética, entre otros factores. Su tratamiento consiste en disminuir los síntomas del paciente y el tiempo de lesión en la cavidad bucal, asociado a fármacos o terapias que aceleren el proceso de reparación y analgesia. La irradiación láser de bajo nivel (LLLT) se ha utilizado y estudiado como terapia auxiliar para RAS. Tiene, entre muchos beneficios, efectos analgésicos, antiinflamatorios y regeneradores. El objetivo de este estudio fue analizar la efectividad de la TLBI en la reparación tisular, analgesia y recurrencia de lesión aftosa mayor recurrente en un paciente hebiátrico seleccionado en la Clínica Odontológica de la Universidad de Mogi das Cruzes con seguimiento desde 2019, en el que la irradiación por contacto con Se realizó onda infrarroja de 808nm y dosis alrededor de 105J/cm2 en 4 puntos intraorales, seguido de aplicación extraoral para drenaje linfático, en dos sesiones con intervalo de 24 horas, con seguimiento a los 60 días ya los 2 años. O seu tratamento consiste na diminuição da sintomatologia do paciente e do tempo da lesão na cavidade bucal, associada a medicamentos ou terapias que acelerem o processo de reparação e analgesia. A irradiação com o laser em baixa intensidade (LBI) tem sido utilizada e estudada como terapia auxiliar para a EAR. Possui, entre diversos prós, efeitos analgésicos, anti-inflamatórios e eficácia regenerativa. O objetivo deste estudo foi analisar a efetividade do LBI na reparação tecidual, analgesia e recorrência da lesão aftosa maior recorrente em um paciente hebiatra selecionado na Clínica Odontológica da Universidade de Mogi das Cruzes acompanhado desde 2019, no qual foi realizada a irradiação de contato com comprimento de onda infravermelho de 808nm e dose em torno de 105J/cm2 em 4 pontos intrabucais, seguida de uma aplicação extrabucal para drenagem linfática, em duas sessões com intervalo de 24 horas, com acompanhamento após 60 dias e após 2 anos. Por meio da escala visual analógica (EVA), o paciente indicava a intensidade e evolução da dor, a qual foi reduzida de 10 (dor intensa) na 1ª sessão para 1 (dor leve) na 2ª sessão e permaneceu em 0 na 3ª sessão, após 60 dias e após 2 anos. Clinicamente, a lesão demorava por volta de 14 dias para cicatrizar e com a irradiação o tempo diminuiu para 3 dias, não havendo recidiva após 2 anos.
