Generations of stents in the treatment of ischemic cardiopathy

Resumo: Coronary Artery Disease predisposes to the development of ischemic heart disease, being one of the main causes of morbidity and mortality worldwide. Percutaneous coronary interventions revascularize the myocardium through the implantation of stents in order to restore blood flow in the affected arteries. Due to the high use of these devices in percutaneous procedures, as well as the currently found limitations, such as stent thrombosis, restenosis and adverse cardiac events, the aim of the present study was to analyze publications that deal with the different generations of stents in the treatment of arterial disease coronary, through a literary review, with a search in Scielo, Pubmed and Sciencedirect databases. Drug-eluting stents have emerged as a safer and more effective alternative than conventional stents, reducing episodes of restenosis and major adverse cardiac events. However, an increased risk of thrombosis linked to its use was observed. Thus, by improving the design, as well as the materials used in its manufacture, several generations of drug-eluting stents were developed, expanding the therapeutic collection. Despite this, further studies are still needed to prove its long-term safety.

Las intervenciones coronarias percutáneas revascularizan el miocardio mediante la implantación de stents para restablecer el flujo sanguíneo en las arterias afectadas. Debido al alto uso de estos dispositivos en los procedimientos percutáneos, así como a las limitaciones encontradas actualmente, como la trombosis del stent, la restenosis y los eventos cardíacos adversos, el objetivo del presente estudio fue analizar publicaciones que aborden las diferentes generaciones de stents en el tratamiento de la enfermedad arterial coronaria, a través de una revisión literaria, con una búsqueda en las bases de datos Scielo, Pubmed y Sciencedirect. Los stents liberadores de fármacos han surgido como una alternativa más segura y efectiva que los stents convencionales, reduciendo los episodios de reestenosis y los principales eventos cardíacos adversos. Sin embargo, se observó un mayor riesgo de trombosis relacionada con su uso. Por lo tanto, al mejorar el diseño, así como los materiales utilizados en su fabricación, se desarrollaron varias generaciones de stents liberadores de fármacos, ampliando la colección terapéutica. A pesar de esto, aún se necesitan más estudios para demostrar su seguridad a largo plazo.A Doença Arterial Coronariana predispõe o desenvolvimento de doenças isquêmicas cardíacas, sendo uma das principais causas de morbimortalidade em todo mundo. As intervenções coronarianas percutâneas revascularizam o miocárdio por meio da implantação de stents a fim de restabelecer o fluxo sanguíneo nas artérias afetadas. Devido à alta no uso desses dispositivos nos procedimentos percutâneos, bem como as limitações atualmente encontradas, como trombose de stent, reestenose e eventos cardíacos adversos, o objetivo do presente estudo foi analisar as publicações que tratam das diversas gerações de stents no tratamento da doença arterial coronariana, através de uma revisão literária, com busca nas bases Scielo, Pubmed e Sciencedirect. Os stents farmacológicos surgiram como uma alternativa mais segura e eficaz que os stents convencionais, reduzindo episódios de reestenose e eventos cardíacos adversos maiores. No entanto, um maior risco de trombose ligada ao seu uso foi observado. Assim, por meio de um aperfeiçoamento no design, bem como dos materiais utilizados na sua fabricação, diversas gerações de stents farmacológicos foram desenvolvidas, ampliando o acervo terapêutico.
