Acute poisoning by organophosphorus pesticides: an integrative review

Resumo: Organophosphate pesticides can cause intoxication in exposed individuals. The objective of this work was to carry out an integrative review on acute human intoxication by organophosphate pesticides. The search for scientific articles was carried out in the CAPES Journals Portal and in the Scopus database using the combination of the descriptors “organophosphates”, “cholinesterase” and “acute intoxication” interrelated by the Boolean localizer “AND”. Works in English and Portuguese, published between 2016 and 2021 were considered. 146 articles were identified and after reading the title and abstract, 26 potentially eligible articles were selected, which were read in full, observing if, in fact, they met the criteria settled down. After reading, 19 articles were selected to compose the review. Studies report the importance of measuring cholinesterases in the assessment of acute poisoning by organophosphate pesticides, however, most perform the dosage without the basal value of these enzymes. Although the Ellman method is the most common form of quantification, there are many methodological variations. Thus, it is essential to study new markers for acute intoxication and to optimize existing methodologies. The symptoms that affect individuals are nonspecific and may be related to the specific active ingredient of each organophosphate. Factors such as lifestyle, presence of metabolic alterations, low education, advanced age, low use of PPE and inappropriate use of pesticides are also associated with cases of intoxication. The training and awareness of individuals who handle organophosphates, in addition to actions by the government, are essential to strengthen regulations on the use of these substances, in order to avoid health problems in the population.

El objetivo de este trabajo fue realizar una revisión integradora sobre la intoxicación humana aguda por plaguicidas organofosforados. La búsqueda de artículos científicos se realizó en el Portal de Revistas de la CAPES y en la base de datos Scopus utilizando la combinación de los descriptores “organofosforados”, “colinesterasa” e “intoxicación aguda” interrelacionados por el localizador booleano “AND”. Se consideraron trabajos en inglés y portugués, publicados entre 2016 y 2021. Se identificaron 146 artículos y después de la lectura del título y el resumen, se seleccionaron 26 artículos potencialmente elegibles, que se leyeron en su totalidad, observando si, de hecho, cumplían con los criterios establecidos. abajo. Factores como el estilo de vida, presencia de alteraciones metabólicas, baja educación, edad avanzada, bajo uso de EPP y uso inadecuado de pesticidas también están asociados a casos de intoxicación. La capacitación y concientización de las personas que manipulan organofosforados, además de las acciones del gobierno, son fundamentales para fortalecer las regulaciones sobre el uso de estas sustancias, a fin de evitar problemas de salud en la población.Agrotóxicos organofosforados podem ocasionar intoxicação nos indivíduos expostos. O trabalho objetivou a elaboração de uma revisão integrativa sobre a intoxicação aguda humana por agrotóxicos organofosforados. A busca foi realizada nas bases de dados do Portal de Periódicos da CAPES e Scopus, utilizando a combinação “organophosphates”, “cholinesterase” e “acute intoxication”. Foram considerados artigos em inglês e português, publicados entre 2016 e 2021. Após a seleção através do título/resumo e leitura completa dos potencialmente elegíveis, foram selecionados 19 artigos. Os estudos relatam a importância da dosagem de colinesterases na avaliação de intoxicação aguda por organofosforados, contudo, a maioria realiza a dosagem sem o valor basal destas enzimas. Apesar do método de Ellman ser o mais comum para a quantificação, existem muitas variações metodológicas. Assim, é fundamental o estudo de novos marcadores e a otimização das metodologias existentes. Os sintomas de intoxicação são inespecíficos e podem estar relacionados ao ingrediente ativo de cada organofosforado. Fatores como estilo de vida, alterações metabólicas, baixa escolaridade, idade avançada, baixo uso de EPIs e uso inadequado de agrotóxicos também estão associados.
