Resumo: | Fractures of the zygomatic bone, when displaced, usually have great clinical repercussions, whether functional or aesthetic. Many of the aesthetic complaints are due to the zygoma being located in a very prominent region on the face. The main functional limitations reported by patients are diplopia, limited mouth opening and paresthesia in the regions innervated by the infraorbital nerve. They are usually caused by physical aggression, traffic accidents or by the victim of a firearm projectile, but very rarely being caused accidentally in the author of the shot. This work aims to report the immediate handling of a rare accident caused by the penetration of a firearm component in the face of the shooter, in which the initial conduct was to remove the object, preserving as much tissue as possible. soft and hard, and a second surgical time for bone fixation after adequate planning, showing the importance of directing the planning according to the patient's complaints. In this way, the professional must have a good knowledge of the main signs and symptoms of facial fractures, in order to provide an adequate diagnosis and surgical planning to the patient, especially in cases of more atypical traumas. |

Muchas de las quejas estéticas se deben a que el cigoma se encuentra en una región muy prominente de la cara. Las principales limitaciones funcionales referidas por los pacientes son diplopía, limitación de la apertura bucal y parestesias en las regiones inervadas por el nervio infraorbitario. Suelen estar provocados por agresiones físicas, accidentes de tráfico o por la víctima de un proyectil de arma de fuego, pero muy rara vez se producen de forma accidental en el autor del disparo. Este trabajo tiene como objetivo relatar el manejo inmediato de un raro accidente causado por la penetración de un componente de arma de fuego en la cara del tirador, en el cual la conducta inicial fue la de retirar el objeto, preservando la mayor cantidad posible de tejido blando y duro, y un segundo tiempo quirúrgico para la fijación ósea después de una planificación adecuada, mostrando la importancia de orientar la planificación de acuerdo con las quejas del paciente. De esta forma, el profesional debe tener un buen conocimiento de los principales signos y síntomas de las fracturas faciales, para poder brindar un adecuado diagnóstico y planificación quirúrgica al paciente, especialmente en los casos de traumatismos más atípicos.As fraturas do osso zigomático, quando deslocadas, costumam apresentar grande repercussão clínica, seja funcional ou estética. Muitas das quieixas estéticas, é decorrente do zigoma estar localizado numa região muito proeminente na face. As principais limitações funcionais relatas pelos pacientes são a diplopia, limitação de abertura bucal e a parestesia nas regiões inervadas pelo nervo infraorbitário. Geralmente são causadas por agressão física, acidentes de trânsito ou por vítima de um projétil de arma de fogo, porém muito raramente sendo causadas de forma acidental no autor do disparo. Este trabalho tem por objetivo relatar o manejo imediato diante de um acidente raro causado pela penetração de um componente de uma arma de fogo na face do autor do disparo, no qual a conduta inicial foi a retirada do objeto, preservando-se o máximo de tecidos moles e duros, e um segundo tempo cirurgico para fixação óssea após planejamento adequado, mostrando a importância de direcionar o planejamento de acordo com as queixas do paciente.
