Resumo: Childhood acute lymphoblastic or lymphoblastic leukemia (ALL) is the result of attack on immature type B lymphocytic cells, the result of acquired genetic damage rather than hereditary, occupying first place in the ranking of disease incidence, according to the National Institute of Cancer (INCA). Improvements in study techniques now provide the opportunity to obtain clinically relevant biological information that may explain apparently abnormal responses to treatment. This study aimed to describe useful diagnostic methods for the classification and treatment of acute lymphoblastic leukemia in children and to present the implications of cytomorphology, cytogenetics, immunophenotyping and cytochemical techniques in the diagnosis and prognosis of leukemia. The systematic review for the elaboration of this consensus was preferably based on indexed articles published in the last ten years based on data from PubMed, Scielo, NCBI. With the results, it was possible to verify, in relation to the hemogram, that the global count of leukocytes in ALL in children can be decreased, normal or increased. In myelogram, the diagnosis of ALL in children is confirmed when more than 25% to 30% of the nucleated cells are blasts. Based on the cytochemical stains, ALL is diagnosed when a positive test is obtained for the PAS techniques. Therefore, knowledge of ways to diagnose and classify ALL in children is extremely important for an early diagnosis and, therefore, to promote an effective treatment.

Las mejoras en las técnicas de estudio brindan ahora la oportunidad de obtener información biológica clínicamente relevante que podría explicar las respuestas aparentemente anómalas al tratamiento. Este estudio tuvo como objetivo describir las formas de diagnóstico útiles para la clasificación y tratamiento de la leucemia linfoblástica aguda en niños y presentar las implicaciones de la citomorfología, citogenética, inmunofenotipificación y técnicas citoquímicas en el diagnóstico y pronóstico de la leucemia. La revisión sistemática para la elaboración de este consenso se basó preferentemente en artículos indexados publicados en los últimos diez años con base en datos de PubMed, Scielo, NCBI. Con los resultados, se pudo ver, en relación al hemograma, el recuento global de leucocitos en LLA en niños puede estar disminuido, normal o aumentado. En el mielograma, el diagnóstico de LLA en niños se confirma cuando más del 25% al ​​30% de las células nucleadas son blastos. Según las tinciones citoquímicas, la LLA se diagnostica cuando se obtiene una prueba positiva para las técnicas de PAS. Por tanto, el conocimiento sobre las formas de diagnóstico y clasificación de la LLA en niños es de suma importancia para un diagnóstico precoz y, por tanto, para promover un tratamiento eficaz.A leucemia linfóide ou linfoblástica aguda (LLA) infantil é decorrente do ataque às células linfóciticas do tipo B imaturos, resultante de danos genéticos adquiridos e não herdados, ocupando o primeiro lugar no ranking de incidência de doenças, segundo o instituto nacional do câncer (INCA). A melhoria nas técnicas de estudo proporciona agora a oportunidade de obter informações biológicas clinicamente relevantes que poderão explicar as respostas aparentemente anômalas ao tratamento. Este estudo teve como objetivo descrever as formas de diagnóstico úteis para a classificação e tratamento de Leucemia Linfoblástica Aguda em Crianças e apresentar as implicações das técnicas de citomorfologia, citogenética, imunofenotipagem e citoquímica no diagnóstico e prognóstico das leucemias. A revisão sistemática para elaboração deste consenso baseou-se preferencialmente em artigos indexados, publicados nos últimos dez anos fundamentados nos dados PubMed, Scielo, NCBI. Com os resultados foi possível perceber em relação ao hemograma, a contagem global de leucócitos na LLA em criançaspode estar diminuída, normal ou aumentada.  Já no mielograma, o diagnóstico da LLA em crianças é confirmado quando mais de 25% a 30% das células nucleadas são blastos. De acordo com as colorações citoquimicas, diagnostica-se LLA quando se obtém uma prova positiva para as técnicas de PAS.
