Association of hepatic steatosis in patients in preoperative bariatric surgery with obstructive sleep apnea

Resumo: Obstructive sleep apnea (OSA) is a global disease that is associated with metabolic syndrome, especially obesity. Intermittent hypoxemia, resulting from OSA, that can lead to an increase in insulin resistance and a change in lipid metabolism, which may precipitate hepatic steatosis, characterized by the accumulation of lipids within the hepatocytes, considered a probable main cause of morbidity and mortality in liver disease. Thus, this retrospective study sought to assess a possible relationship between OSA and hepatic steatosis in obese patients in the preoperative period for bariatric surgery. From the data collection of 1,416 medical records of patients undergoing preoperative evaluation, data from ultrasound examination (US) and polysomnography (PSG) were obtained, resulting in a sample of 87 individuals. Of this sample, 61% of the individuals were female, with an average age of 39 years old (SD 11.4) and a BMI of 39.3 (SD 4,4). Ultrasonography classified 74 individuals with hepatic steatosis (85%). The average value of the Apnea-Hypopnea Index (AHI) was 33.5 (SD 28.9) and the Oxyhemoglobin Desaturation Index (ODI) 41.4 (SD 34.1). The patients were divided into 2 groups: a group with AHI less than or equal 15 events / hour and another with AHI greater than or equal 16 events / hour. The degree of steatosis differs between groups by AHI (p <0.001) and gender (p <0.001). After adjusting several variables using multiple regression, only the male sex was a predictor of higher steatosis degree. In conclusion there is a higher prevalence of hepatic steatosis among patients with OSA, and the main factors associated with steatosis were the AHI and male sex, but after regression only female sex would be the steatosis protection predictor in the study group.

La hipoxemia intermitente, resultante de la AOS, puede provocar un aumento de la resistencia a insulina y un cambio en el metabolismo de los lípidos, lo que puede precipitar la esteatosis hepática, caracterizada por la acumulación de lípidos en el interior de los hepatocitos, considerada una probable causa principal de morbilidad y mortalidad asociada a dolencias del hígado. Se trata de un estudio retrospectivo que busca evaluar una posible relación entre la AOS y la esteatosis hepática en pacientes obesos en el período preoperatorio de cirugía bariátrica. A partir de 1416 registros médicos de pacientes sometidos a evaluación preoperatoria, se obtuvieron datos de exámenes de ultrasonido (USG) y polisomnografía (PSG), resultando una muestra de 87 individuos evaluados. De esta muestra, 61% de los individuos eran mujeres, con una edad media de 39 años, con una desviación estándar (DE) de 11,4 y un IMC de 39,3 (DE de 4,4). La ecografía clasificó a 74 individuos con esteatosis hepática (85%). El valor medio del índice de apnea-hipopnea (IAH) fue de 33,5 (DE 28,9) y el índice de desaturación de oxihemoglobina (IDO) fue de 41,4 (DE 34,1). Los pacientes fueron divididos en 2 grupos: un grupo con IAH menor o igual a 15 eventos/hora y el otro con IAH mayor o igual a 16 eventos/hora. El grado de esteatosis difirió entre los grupos por IAH (p<0,001) y sexo (p<0,001). Después de ajustar las variables mediante regresión múltiple, solo el sexo masculino fue un predictor de grados más elevados de esteatosis. Concluimos que existe una mayor prevalencia de esteatosis hepática entre los pacientes con AOS, y los principales factores asociados a la esteatosis fueran el IAH y sexo masculino, todavía por regresión solo el sexo femenino sería predictor de protección de esteatosis en la muestra estudiada.A apneia obstrutiva do sono (AOS) é uma doença global que se associa à síndrome metabólica, especialmente à obesidade. A hipoxemia intermitente decorrente da AOS pode levar a um aumento na resistência à insulina e a uma alteração no metabolismo lipídico, podendo precipitar o desenvolvimento de esteatose hepática, caracterizada pelo acúmulo de lipídeos no interior dos hepatócitos, considerada provável causa principal de morbidade e mortalidade ligada a doenças do fígado. Dessa forma, este estudo retrospectivo buscou avaliar uma possível relação entre a AOS e a esteatose hepática em pacientes obesos no pré-operatório para cirurgia bariátrica. A partir da coleta de dados de 1.416 prontuários de pacientes em avaliação pré-operatória, obteve-se dados dos exames de ultrassonografia (USG) e polissonografia (PSG), resultando em uma amostra de 87 indivíduos avaliados. Desta amostra, 61% dos indivíduos eram do sexo feminino, com idade média de 39 anos de idade, com desvio padrão (d.p.) de 11,4 e IMC de 39.3 (d.p. 4,4). A ultrassonografia classificou como portadores de esteatose hepática 74 indivíduos (85%). O valor médio do Índice de apneia-hipopneia (IAH) foi de 33.5 (d.p. 28.9) e do Índice de dessaturação da oxihemoglobina (IDO) foi de 41.4 (d.p. 34,1). Os pacientes foram divididos em 2 grupos: um grupo com IAH menor ou igual 15 eventos/hora e outro com IAH maior ou igual 16 eventos/hora. O grau de esteatose diferiu entre os grupos pelo IAH (p <0,001) e sexo (p <0,001).  Após o ajuste de várias variáveis por meio de regressão múltipla, apenas o sexo masculino foi um preditor de graus mais elevados de esteatose.
