Histopathological findings obtained by uterine curettage due to abortion at the Santa Isabel Maternity (Aracaju-Sergipe-Brazil) in the 2015-2017 period

Resumo: Abortion is a common event that directly affects the family, social and health spheres. It is defined by the World Health Organization (WHO) as the expulsion or extraction of the fetus before 20 weeks or weighing less than 500g and has well-defined clinical and histopathological characteristics, ranging from benign conditions to life-threatening pathologies. Aiming to outline the profile of histopathological findings by uterine curettage for abortion in a maternity hospital in the city of Aracaju, State of Sergipe, between 2015 and 2017, a total of 2473 findings were analyzed. Of these, the majority corresponded to incomplete abortion (98.83%) and to products of conception (61.91%). Seven cases of hydatidiform mole were found – four incomplete and three complete. In the profile of the population studied, 91.14% of women did not have partners and 88.31% were single. The abortion rate per ten thousand inhabitants was also analyzed, drawing attention to the fact that the greater Aracaju concentrates a number about four times greater than other municipalities in the state of Sergipe. There is a lot of debate about the high costs of performing routine uterine curettage with histopathological analysis, since the risk of developing gestational trophoblastic disease is low, corresponding to 0.003% in the present study. Although this is true, when comparing the clinical aspect of the abortion with the histopathological findings, a p of 0.23 was found, and it was not possible to reliably establish a clinical profile for potentially malignant conditions.

Se define por la Organización Mundial de la Salud (OMS) como la expulsión o extracción del feto antes de las 20 semanas o con un peso inferior a 500 gy tiene características clínicas e histopatológicas bien definidas, que van desde condiciones benignas hasta patologías potencialmente mortales. Con el objetivo de delinear el perfil de hallazgos histopatológicos mediante legrado uterino para aborto en una maternidad de la ciudad de Aracaju, Estado de Sergipe, entre 2015 y 2017, se analizaron un total de 2473 hallazgos. De estos, la mayoría correspondió a abortos incompletos (98,83%) y productos de la concepción (61,91%). Se encontraron siete casos de mola hidatiforme, cuatro incompletos y tres completos. En el perfil de la población estudiada, el 91,14% de las mujeres no tenían pareja y el 88,31% eran solteras. También se analizó la tasa de abortos por cada diez mil habitantes, destacando que el Gran Aracaju concentra un número cuatro veces mayor que otros municipios del estado de Sergipe. Existe un gran debate sobre los altos costos de realizar un legrado uterino de rutina con análisis histopatológico, ya que el riesgo de desarrollar enfermedad trofoblástico gestacional es bajo, correspondiente al 0,003% en el presente estudio. Si bien esto es cierto, al comparar el aspecto clínico del aborto con los hallazgos histopatológicos se encontró una p de 0.23, y no fue posible establecer de manera confiable un perfil clínico de patologías potencialmente malignas.O abortamento é um evento comum que repercute diretamente no âmbito familiar, social e da saúde.  É definido pela Organização Mundial de Saúde (OMS) como a expulsão ou extração do feto antes de 20 semanas ou pesando menos de 500g e possui características clínicas e histopatológicas bem definidas, passando desde condições benignas até patologias ameaçadoras à vida. Com o objetivo de traçar o perfil dos achados histopatológicos por curetagem uterina por abortamento em uma maternidade do Município de Aracaju, Estado de Sergipe, entre 2015 a 2017, foram analisados um total de 2473 achados. Destes, a grande parte correspondeu a o aborto incompleto (98,83%) e a produtos de concepção (61,91%). Foram encontrados sete casos de mola hidatiforme – quatro de mola incompleta e três de completa. No perfil da população estudada, 91,14% das mulheres não possuíam parceiros e 88,31% eram solteiras. Também foi analisada a taxa de abortamento por dez mil habitantes, chamando atenção o fato de a grande Aracaju concentrar um número cerca de quatro vezes maior que os demais munícipios do estado de Sergipe. Muito se discute sobre os custos elevados da realização de curetagem uterina rotineira com análise histopatológica, visto que o risco de desenvolver doença trofoblástica gestacional é baixo, correspondendo a 0,003% no presente estudo.
