Correlation between bruxism and pain of endodontic origin: a case report

Resumo: The objective was to report a case of endodontic treatment in a patient diagnosed with irreversible pulpitis presenting as a causative and aggravating factor severe bruxism. Patient G. L. A., a 55-year-old, military police officer, attended the clinic complaining of “toothache”, characterized by being localized, of long duration, moderate intensity and caused by thermal and masticatory stimuli. During the anamnesis, the patient reported to be allergic to dipyrone sodium and hypertensive, making use of Captopril®. During the intra-oral physical examination, was observed that the complaint was regarding the element 34, which did not present fistula. The patient had a history of severe bruxism, presenting extensive occlusal wear of all the dental elements and made use of a myorelaxing plate. Percussion and pulpar vitality tests were performed, reaching a final diagnosis of irreversible pulpitis, making it necessary to perform conventional pulpectomy as treatment. The endodontic treatment performed successfully attended the patient’s needs, starting with the pain relief and, so that the stomatognathic system could also be restored, he was referred to the clinic for oral rehabilitation.

Paciente del sexo masculino de 55 años, policia, fué atendido en la clínica, quejándose de dolor de diente, caracterizado por ser de larga duración, de intensidade moderada y causada por estímulos térmicos y masticatórios. El paciente informo ser alérgico a la dipirona sódica y además que era hipertenso y hacia uso de Captopril®.  Al realizar el examen clínico se observó que la queja estaba relacionada con el elemento 34, el cual no presentaba fistula. El paciente tenía bruxismo severo, presentando un extenso desgaste en la superficie oclusal de todos los elementos dentales y usaba una placa miorelajante. Se realizaron pruebas de percusión y vitalidad pulpar. Fue diagnosticada una pulpite irreversible, habiendo sido necesaria la pulpectomia como tratamiento convencional. El tratamiento endodóntico fué realizado con éxito, aliviando el dolor y para que el sistema estomatognático del paciente fuera reestablecido fué encaminhado para la clínica de reabilitación oral.Palabras clave: enfermedades periodontales, ácido cítrico, ácido edético, periodoncia.O objetivo foi relatar um caso de tratamento endodôntico em um paciente diagnosticado com pulpite irreversível apresentando como fator causador e agravante um bruxismo severo. Paciente G. L. A., gênero masculino, 55 anos, policial militar, compareceu à clínica se queixando de “dor de dente”, caracterizada por ser localizada, de longa duração, intensidade moderada e causada por estímulos térmico e mastigatório. Na anamnese, o paciente relatou ser alérgico à dipirona sódica e ser hipertenso, fazendo uso de Captopril®. Durante o exame físico intra-oral, observou-se que a queixa era referente ao elemento 34, o qual não apresentava fístula. O paciente era portador de um bruxismo severo, apresentando extensos desgastes na superfície oclusal de todos os elementos dentários e fazia uso de placa miorrelaxante. Foram realizados os testes de percussão e vitalidade pulpar chegando ao diagnóstico final de pulpite irreversível, fazendo-se necessário a pulpectomia convencional como tratamento.
