Dysphagia and gastresophageal reflux in institutionalized elderly

Resumo: Objective: The aim of this study was to identify the prevalence of gastroesophageal reflux and dysphagia in institutionalized elderly people. Method: This is a retrospective, analytical, quantitative documentary study, in which medical records of 191 institutionalized elderly people living in five long- stay institutions located in the city of João Pessoa-PB were consulted. The instrument used for data collection from medical records was a form elaborated for this purpose, adapted from the triage protocols RADI (Screening of Elder Oropharyngeal Dysphagia) and SSI (Index of Reflux Symptoms). The statistical analysis was performed to obtain measures of central and inferential tendency through the correlation tests and the Kolmogorov-Smirnov test was applied to test the normality of the sample distribution. To analyze the association  between the two dependent variables, having or not dysphagia and having or not reflux, Pearson’s chi-square tests or Fisher's exact text were applied. Results: Most of the study sample was composed of women (n=144 / 75.4%); among the elderly in the study group, the majority presented with some food refusal, delay to eat and difficulty to eat some food, and, the higher the age, the greater the presence of food refusal and weight loss in this age group. Conclusion: The elderly in the study showed a low presence of gastroesophageal reflux symptoms, with some associated signs and symptoms of dysphagia.

Método: Se trata de un estudio documental retrospectivo, analítico y cuantitativo, en el que se consultaron las historias clínicas de 191 ancianos institucionalizados que viven en las cinco instituciones de atención a largo plazo ubicadas en la ciudad de João Pessoa-PB. El instrumento utilizado para la recogida de datos en las historias clínicas fue un formulario elaborado para este fin, adaptado a partir de los protocolos de cribado RADI (cribado de disfagia orofarina en ancianos) y SSI (índice de síntomas de reflujo). Se realizó un análisis estadístico para obtener medidas de tendencia central e inferencial a través de pruebas de correlación y se aplicó la prueba de Kolmogorov-Smirnov para probar la normalidad de la distribución de la muestra. Se aplicaron las pruebas de chi-cuadrado de Pearson o de texto exacto de Fisher para analizar la asociación entre las dos variables dependientes o no tener disfagia y tener o no reflujo. Resultados: La mayor parte de la muestra del estudio consistió en mujeres (n 144 / 75.4%), entre los ancianos, la mayoría se negó a algunos alimentos, mientras que la minoría informó una caída en los alimentos en la boca en cuánto, la mayoría de los ancianos informaron retraso en la alimentación y dificultad para comer algún alimento, se observó que cuanto mayor era la presencia de rechazo de alimentos y pérdida de peso en esta población. Conclusión: Se observó que los ancianos en el estudio mostraron una baja presencia de síntomas de reflujo gastroesofágico, con algunos signos y síntomas de disfagia asociada.Objetivo: O objetivo deste estudo foi identificar a prevalência de refluxo gastresofágico e disfagia em idosos institucionalizados. Método: Trata-se de um estudo documental retrospectivo, analítico, quantitativo, no qual foram consultados prontuários de 191 idosos institucionalizados residentes nas cinco instituições de longa permanência localizadas na cidade de João Pessoa-PB. O instrumento utilizado para a coleta de dados nos prontuários foi um formulário elaborado para esse fim, adaptado a partir dos protocolos de triagem RADI (Rastreamento de disfagia orofaríngea em idosos) e ISR (índice de sintomas de refluxo). A análise estatística foi realizada para obtenção de medidas de tendência central e inferencial por meio dos testes de correlação e o teste de Kolmogorov-Smirnov foi aplicado para testar a normalidade da distribuição da amostra. Para análise da associação entre as duas variáveis dependentes ter ou não disfagia e ter ou não refluxo, foram aplicados os testes qui-quadrado de Pearson ou Texto exato de Fisher. Resultados: Maior parte da amostra do estudo foi composta por mulheres (n 144 / 75,4%), dentre os idosos a maioria apresentou recusa ao algum alimento, ao mesmo tempo em que a minoria relatou queda de alimento da boca em quanto, a maioria dos idosos relatou demora a se alimentar e dificuldade para comer algum alimento, observou-se que quanto maior a idade maior a presença de recusa alimentar e perda de peso nessa população.
