Surgical approach in Temporomandibular Joint disorders (TMJ): a literature review

Resumo: The temporomandibular joint (TMJ) is a highly specialized mobile joint, considered one of the most complex and used in the human body. Surgery for temporomandibular joint disorders may include mandibular manipulation assisted with increased hydrostatic pressure, astrocentesis, arthroscopy, arthrotomy, eminoplasty, and eminectomy. The aim of this study is to carry out a literature review on the different surgical techniques in temporomandibular joint disorders and their indications, in order to guide the dentist to further clarification and, consequently, to a better approach. Data were collected through databases such as: PubMed, Scientific Electronic Library Online (SciELO) and academic google, from February to April 2021. The keywords were used: temporomandibular joint, temporomandibular joint anatomy, disorders of the temporomandibular joint, temporomandibular joint pathology, temporomandibular joint disorders, TMD treatment, TMD surgery. Thus, the study is very relevant because without an intervention, TMJ disorders can worsen and a correct diagnosis is essential for the correct surgical approach to be applied.

La cirugía para los trastornos de la articulación temporomandibular puede incluir manipulación mandibular asistida con aumento de la presión hidrostática, astrocentesis, artroscopia, artrotomía, eminoplastia y eminectomía. El objetivo de este estudio es realizar una revisión de la literatura sobre las diferentes técnicas quirúrgicas en los trastornos de la articulación temporomandibular y sus indicaciones, con el fin de orientar al odontólogo a una mayor aclaración y, en consecuencia, a un mejor abordaje. Los datos se recolectaron a través de bases de datos como: PubMed, Scientific Electronic Library Online (SciELO) y Google académico, de febrero a abril de 2021. Se utilizaron las palabras clave: articulación temporomandibular, anatomía de la articulación temporomandibular, trastornos de la articulación temporomandibular, patología de la articulación temporomandibular, temporomandibular trastornos de las articulaciones, tratamiento de TMD, cirugía de TMD. Así, el estudio es muy relevante porque sin una intervención los trastornos de la ATM pueden empeorar y un diagnóstico correcto es fundamental para aplicar el abordaje quirúrgico correcto.A articulação temporomandibular (ATM) é uma articulação móvel, altamente especialidazada, sendo considerada uma das mais complexas e utilizadas do organismo humano. A cirurgia de desordens da articulação temporomandibulares pode incluir, manipulação mandibular assistida com aumento de pressão hidrostática, astrocentese, artroscopia, artrotomia, eminoplastia e eminectomia. Objetivo desse estudo é realizar uma revisão de literatura sobre as diferentes técnicas cirúrgicas em desordens da articulação temporomandibular e suas indicações, de modo a orientar o cirurgião dentista a um maior esclarecimento e consequentemente a uma melhor conduta. Os dados foram levantados através de bases de dados como: PubMed, Scientific Electronic Library Online (SciELO) e google acadêmico, no período de fevereiro a abril de 2021. Foram  utilizdas as palavras-chave: articulação temporomandibular, anatomia da articulação temporomandibular, desordens da articulação temporomandibular, patologia da articulação temporomandibular, transtornos da articulação temporomandibular, tratamento de DTM, cirurgia de DTM.
