Segmental osteotomy of the maxilla in the aesthetic area to enable rehabilitation with dental implants: case report

Resumo: Currently, the success of rehabilitation with implants has aroused in patients the interest and hope to smile again after having undergone tooth extractions in the past, and for the fact of not adapting to the use of conventional dental prostheses for a long time. The segmental maxillary osteotomy procedure is a viable treatment proposal for patients who present a decrease in the anterior vertical dimension of the maxilla, due to the chronic resorptive process in the region, making implant-supported rehabilitation more satisfactory. In view of the need to correct maxillary discrepancies in a single surgical procedure, some aspects must be taken into account, such as: patient selection, surgical and prosthetic planning and the determination of a more predictable type of treatment, from the patient's perspective. The objective of this work is to report a clinical case of a patient who underwent segmental osteotomy en bloc of the anterior region of the maxilla, prior to implant-supported rehabilitation. A female patient came to the service complaining of discomfort due to using dental prosthesis for a long time. Upon initial clinical and imaging examination, the need to increase bone height in the anterior region of the maxilla was found. It was proposed for the case to perform a segmental osteotomy of the anterior region of the maxilla to gain bone height in the region that allowed a functional and aesthetic rehabilitation for the patient. With this case, it can be concluded that the block segmental osteotomy procedure of the anterior region of the maxilla for aesthetic correction is effective and requires adequate planning to obtain satisfactory aesthetic results.

El procedimiento de osteotomía maxilar segmentaria es una propuesta de tratamiento viable para los pacientes que presentan una disminución en la dimensión vertical anterior del maxilar, debido al proceso crónico de resorción en la región, haciendo más satisfactoria la rehabilitación implantosoportada. Ante la necesidad de corregir las discrepancias maxilares en un solo procedimiento quirúrgico, se deben tener en cuenta algunos aspectos como: la selección del paciente, la planificación quirúrgica y protésica y la determinación de un tipo de tratamiento más predecible, desde la perspectiva del paciente. El objetivo de este trabajo es reportar un caso clínico de un paciente que fue sometido a una osteotomía segmentaria en bloque de la región anterior del maxilar, antes de la rehabilitación implantosoportada. Una paciente acudió al servicio quejándose de molestias por el uso prolongado de prótesis dentales. Tras el examen clínico y de imagen inicial, se encontró la necesidad de aumentar la altura del hueso en la región anterior del maxilar. Se propuso para el caso realizar una osteotomía segmentaria de la región anterior del maxilar para ganar altura ósea en la región que permitiera una rehabilitación funcional y estética del paciente. Con este caso, se puede concluir que el procedimiento de osteotomía segmentaria en bloque de la región anterior del maxilar para corrección estética es eficaz y requiere una planificación adecuada para obtener resultados estéticos satisfactorios.Atualmente o sucesso das reabilitações com implantes tem despertado nos pacientes o interesse e a esperança de sorrir novamente após terem sido submetidos a exodontias no passado, e pelo fato da não adaptação ao uso de próteses dentárias convencionais por muito tempo. O procedimento de osteotomia segmentar da maxila é uma proposta de tratamento viável para pacientes que apresentam diminuição da dimensão vertical anterior da maxila, devido ao processo reabsortivo crônico na região, possibilitando a reabilitação implantossuportada de forma mais satisfatória. Diante da necessidade de se corrigir as discrepâncias maxilares em um único tempo cirúrgico, alguns aspectos devem ser levados em consideração, como: seleção do paciente, planejamento cirúrgico e protético e a determinação de um tipo de tratamento mais previsível, dentro das perspectivas do paciente. O objetivo desse trabalho é relatar um caso clínico de uma paciente submetida a osteotomia segmentar em bloco da região anterior da maxila, prévia a reabilitação implantossuportada. Paciente do sexo feminino procurou o serviço queixando-se de desconforto por usar prótese dentária há muito tempo. Ao exame clínico e imaginológico inicial foi constatada a necessidade de aumentar a altura óssea da região anterior da maxila. Foi proposto para o caso a realização de uma osteotomia segmentar da região anterior da maxila para ganho de altura óssea na região que permitiu uma reabilitação funcional e estética para a paciente.
