The importance of early diagnosis in the elderly patients with Covid-19

Resumo: Introduction: Delirium is a transient neuropsychiatric syndrome considered a geriatric emergency, related to prolonged periods of hospitalization and which contributes to an increase in the mortality rate, especially in the elderly affected by SARS-CoV-2. Objective: To carry out an integrative review of the literature in order to list factors of importance of screening, anamnesis and physical examination to contain the early manifestation of delirium in elderly patients affected by Covid-19. Methodology: This is an integrative literature review carried out by searching for scientific articles in Pubmed (NCBI), Latin American and Caribbean Health Sciences (Lilacs) and Scientific Electronic Library Online (SciELO) databases.). For this, the descriptors were used: Coronavirus, Delirium, Elderly and the filter “PUBLICATION DATE” was selected with the option “5 YEARS”, in the aforementioned databases. The articles selected through the PICO strategy were submitted to methodological rigor and level of evidence analysis. Results: 155 scientific articles were found; however, 17 articles were selected after analyzing the inclusion and exclusion criteria. Of this sample, 41, 17% of the selected studies reported that delirium is associated with high mortality and morbidity, in addition to being often underreported. Screening using tools such as the Chart-Based Delirium Identification Instrument is essential for early detection of delirium. Conclusion: Multiprofessional team work aligned with medical work is necessary for early intervention of delirium in elderly patients with Covid-19.

Objetivo: Realizar una revisión integradora de la literatura con el fin de enumerar factores de importancia del tamizaje, anamnesis y examen físico para contener la manifestación temprana de delirio en pacientes adultos mayores afectados por Covid-19. Metodología: Se trata de una revisión integrativa de la literatura realizada mediante la búsqueda de artículos científicos en las bases de datos Pubmed (NCBI), Latin American and Caribbean Health Sciences (Lilacs) y Scientific Electronic Library Online (SciELO). Para ello se utilizaron los descriptores: Coronavirus, Delirium, Elderly y se seleccionó el filtro “FECHA DE PUBLICACIÓN” con la opción “5 AÑOS”, en las bases de datos antes mencionadas. Los artículos seleccionados a través de la estrategia PICO fueron sometidos a análisis de rigor metodológico y nivel de evidencia. Resultados: se encontraron 155 artículos científicos, sin embargo, después de analizar los criterios de inclusión y exclusión, se seleccionaron 17 artículos. De esta muestra, el 41,17% de los estudios seleccionados informaron que el delirio se asocia con alta mortalidad y morbilidad, además de ser muchas veces infranotificado. La detección mediante herramientas como el Instrumento de identificación del delirio basado en gráficos es esencial para la detección temprana del delirio. Conclusión: El trabajo en equipo multiprofesional alineado con el trabajo médico es necesario para la intervención temprana del delirio en pacientes adultos mayores con Covid-19.Introdução: O delirium é uma síndrome neuropsiquiátrica transitória considerada uma emergência geriátrica, relacionada com períodos prolongados de hospitalização e que contribui para o aumento da taxa de mortalidade, principalmente em idoso acometido pelo SARS-CoV-2. Objetivo: Realizar uma revisão integrativa da literatura com o intuito de elencar fatores de importância da triagem, anamnese e exame físico para conter a manifestação precoce de delirium em pacientes idosos acometidos pela Covid-19. Metodologia: Trata-se de uma revisão integrativa de literatura realizada por meio da busca de artigos científicos nas bases de dados Pubmed (NCBI), Literatura Latino-Americana e do Caribe em Ciências da Saúde (Lilacs) e na Scientific Eletronic Library Online (SciELO). Para isso, foram utilizados os descritores: Coronavírus, Delirium, Elderly e selecionado o filtro “DATA DE PUBLICAÇÃO” com a opção “5 ANOS”, nas bases de dados citadas anteriormente. Os artigos selecionados por meio da estratégia PICO foram submetidos à análise de rigor metodológico e de nível de evidência. Resultados: Foram encontrados 155 artigos científicos, no entanto, selecionou-se 17 artigos após a análise dos critérios de inclusão e exclusão. Dessa amostra, 41, 17% dos estudos selecionados relataram que o delirium está associado a alta mortalidade e morbidade, além de ser, muitas vezes, subnotificado. A realização da triagem por meio de ferramentas como o Chart-Based Delirium Identification Instrument são indispensáveis para detecção precoce de delirium.
