Neutrophil-lymphocyte ratio as a possible prognostic marker in central nervous system tumors: an integrative literature review

Resumo: Introduction: Gliomas are primary tumors originating in the brain parenchyma. Recent studies explore the role of some leukocytes and other markers of inflammation in clinical outcomes associated with the development and progression of this cancer, among them there is the neutrophil per lymphocyte ratio (NLR). Aim: The purpose of this study is to understand, through studies published in the literature, the quantitative neutrophil/lymphocyte ratio and its implications for the prognosis of patients. Methodology: Integrative literature review on the PubMed database of productions from the last 11 years. The descriptors used were: "glioma", "glioblastoma", "NLR", "neutrophil to lymphocyte ratio", "neutrophil lymphocyte ratio", "neutrophil-to-lymphocyte ratio", "prognostic", "prognosis" and "outcome". Results: Sixteen articles were selected at the end of the search steps. The year 2019 being the most prevalent with 31.25% of the publications, followed by the years 2017 and 2018 appearing each year with 18.75%. Regarding the countries of origin of the studies, 50% were conducted in China, followed by the studies conducted in Turkey with 25%. Therefore, it is assertive that a high NLR rate suggests a worse prognosis in patients with gliomas. This is based on two important considerations: (1) the rate varies according to the grade of the glioma, with high grade gliomas (III and IV) having in most cases higher NLR than low grade gliomas (I and II) and (2) the overall survival of individuals with low NLR is considerably higher than in those with high. Final Considerations: NLR has important value as a low-cost and reliable prognostic biomarker, however, further studies are needed seeking to obtain a common NLR cutoff point.

Objetivo: Este trabajo tiene como objetivo comprender, a través de estudios publicados en la literatura, las relaciones cuantitativas de los neutrófilos/linfócitos y los desdoblamientos en el pronóstico de los pacientes. Metodología: Revisión integrativa de la literatura en base a los datos PubMed de las producciones de los últimos 11 años. Los descriptores utilizados fueron: "glioma", "glioblastoma", "NLR", "neutrophil to lymphocyte ratio", "neutrophil lymphocyte ratio", "neutrophil-to-lymphocyte ratio", "prognostic", "prognosis" y "outcome". Resultados: Al final de las etapas de búsqueda se seleccionaron dieciséis artículos. Siendo el año 2019 el más prevalente con el 31,25% de las publicaciones, seguido de los años 2017 y 2018 que aparecen cada uno con el 18,75%. En cuanto a los países de origen de los estudios, el 50% se realizó en China, seguido de los estudios realizados en Turquía con un 25%. Por lo tanto, se puede afirmar que una tasa elevada de NLR sugiere un peor pronóstico en pacientes con gliomas. Esta afirmación se basa en dos consideraciones importantes: (1) la tasa varía según el grado del glioma, teniendo los gliomas de alto grado (III y IV), en la mayoría de los casos, un RNL más alto que los gliomas de bajo grado (I y II) y (2) la supervivencia global de los individuos con un RNL bajo es considerablemente más alta que en los de alto. Consideraciones finales: el NLR tiene un valor importante como biomarcador pronóstico de bajo coste y fiable, sin embargo, es necesario que se realicen más estudios buscando obtener un punto de corte común del NLR.Introdução: Os gliomas são tumores primários originados no parênquima cerebral. Estudos recentes exploram o papel de alguns leucócitos e outros marcadores de inflamação nos resultados clínicos associados ao desenvolvimento e progressão deste câncer, dentre eles há a taxa neutrófilo por linfócito (NLR). Objetivo: Este trabalho tem como propósito compreender, através de estudos publicados na literatura, as relações quantitativas de neutrófilos/linfócitos e os desdobramentos no prognóstico dos pacientes. Metodologia: Revisão Integrativa da literatura na base dados PubMed das produções dos últimos 11 anos. Os descritores utilizados foram: “glioma”, “glioblastoma”, “NLR”, “neutrophil to lymphocyte ratio”, “neutrophil lymphocyte ratio”, “neutrophil-to-lymphocyte ratio”, “prognostic”, “prognosis” e “outcome”. Resultado: Foram selecionados 16 artigos ao final das etapas de busca. Sendo o ano de 2019 o mais prevalente com 31.25% das publicações, seguido pelos anos de 2017 e 2018 que constam cada ano com 18.75%. Em relação aos países de origem dos estudos, 50% foram conduzidos na China, seguido pelos estudos realizados na Turquia com 25%. Por conseguinte, é assertivo que uma alta taxa de NLR sugere um pior prognóstico em pacientes com gliomas. Tal afirmativa parte de duas considerações importantes: (1) a taxa varia de acordo com o grau do glioma, sendo que gliomas de alto grau (III e IV) têm, na maioria dos casos, NLR maior que os gliomas de baixo grau (I e II) e (2) a sobrevivência geral dos indivíduos com baixo NLR é consideravelmente maior que naqueles com alto.
