Placebo neuromodulation effect on fibromyalgia symptoms: a case report

Resumo: Fibromyalgia is a disease in which drug treatment is still not very effective. An alternative that has shown effectiveness in research with chronic pain is transcranial direct current stimulation. An advantage of studies with this technique is the possibility of using the sham protocol, which does not trigger therapeutic effects. Thus, we sought to evaluate the placebo effect of neuromodulation on the levels of pain, functionality, anxiety and depression in a woman with fibromyalgia. This is a case report of female participant, 29 years old, diagnosed with fibromyalgia 17 months ago. The evaluation instruments were the sociodemographic and clinical questionnaire, the Fibromyalgia Impact Questionnaire to evaluate the functionality, the Visual Analogue Scale to check the level of pain, the Beck Anxiety Inventory and the Beck Depression Inventory, to assess anxiety and depression symptoms, respectively. The treatment consisted of five consecutive sessions of placebo type stimulation, with placement of the anodic electrode on the left primary motor cortex and cathodic over the right supraorbital region for 20 minutes. The equipment was automatically turned off after 30 seconds, and did not trigger therapeutic effects. After the revaluations, there was a reduction in pain level from 9.5 to 0, improvement in functionality, with reduction of 65.68 to 14.99 in the Fibromyalgia Impact Questionnaire, and reduction of anxiety and depression levels of 27 to 11 to 25 for 2, respectively. In this sense, it is concluded that the expectation of therapeutic results may lead to clinical improvement in fibromyalgia.

Una alternativa que ha demostrado efectividad en la investigación con dolor crónico es la estimulación transcraneal de corriente continua. Una ventaja de los estudios con esta técnica es la posibilidad de utilizar el protocolo simulado, que no desencadena efectos terapéuticos. Por lo tanto, buscamos informar el efecto placebo de la neuromodulación en los niveles de dolor, funcionalidad, ansiedad y depresión en una mujer con fibromialgia. Es un caso clínico de una participante femenina, de 29 años, diagnosticada con fibromialgia hace 17 meses. Los instrumentos de evaluación fueron el cuestionario sociodemográfico y clínico, el cuestionario de impacto de fibromialgia para evaluar la funcionalidad, la escala visual analógica, para verificar el nivel de dolor, el inventario de ansiedad de Beck y el inventario de depresión de Beck, para evaluar los síntomas de ansiedad. y depresión, respectivamente. El tratamiento consistió en cinco sesiones consecutivas de estimulación de tipo placebo, con la colocación del electrodo anódico sobre la corteza motora primaria izquierda y el cátodo sobre la región supraorbital derecha durante 20 minutos. El equipo se apagó automáticamente después de 30 segundos, sin efectos terapéuticos. Después de las consultas, hubo una reducción en el nivel de dolor de 9.5 a 0, una mejora en la funcionalidad, con una reducción de 65.68 a 14.99 en el Cuestionario de Impacto de Fibromialgia, y una reducción en los niveles de ansiedad y depresión, de 27 a 11 y 25 a 2, respectivamente. En este sentido, se concluye que la expectativa del resultado terapéutico puede ser una posibilidad que resulte en una mejoría clínica en la fibromialgia.A fibromialgia é uma doença na qual o tratamento medicamentoso ainda é pouco eficaz. Uma alternativa que tem mostrado eficácia em pesquisas com dores crônicas é a estimulação transcraniana por corrente contínua. Uma vantagem de estudos com esta técnica é a possibilidade de utilização do protocolo sham, o qual não desencadeia efeitos terapêuticos. Assim, o objetivo foi relatar o efeito placebo da neuromodulação nos níveis de dor, funcionalidade, ansiedade e depressão em uma mulher com fibromialgia. Tratou-se de um relato de caso de uma participante do sexo feminino, 29 anos, diagnosticada com fibromialgia há 17 meses. Os instrumentos avaliativos foram o questionário sociodemográfico e clínico, o Questionário de Impacto da Fibromialgia para avaliar a funcionalidade, a Escala visual analógica, para verificar o nível de dor, o Inventário Beck de Ansiedade e o Inventário Beck de depressão, para avaliar sintomas de ansiedade e depressão, respectivamente. O tratamento constou em cinco sessões consecutivas de estimulação do tipo placebo, com a colocação do eletrodo anódico sobre o córtex motor primário esquerdo e catódico sobre a região supraorbital direita por 20 minutos. O equipamento foi desligado automaticamente após 30 segundos, não desencadeando efeitos terapêuticos. Após os atendimentos, houve redução no nível de dor de 9,5 para 0, melhora na funcionalidade, com redução de 65,68 para 14,99 no Questionário de Impacto da Fibromialgia, e redução dos níveis de ansiedade e depressão, de 27 para 11 e de 25 para 2, respectivamente.
