Main risk factors for lower limb amputation in patients with diabetic foot: a systematic review

Resumo: | Objective: To analyze the main risk factors for lower limb amputation in patients with diabetic foot. Methodology: This is a systematic review carried out using the PRISMA method, in the electronic databases PubMed, Scopus and Web of Science, using the following descriptors: “limb amputation”, “secondary amputation”, “repeat amputation”, “diabetic foot” and “neuropathy”. Full texts, available in Portuguese, English or Spanish, published between January 2016 and December 2021 were included; studies that focused on factors associated with lower limb amputation in patients with diabetic foot; studies that included diabetic patients over 18 years of age; studies whose factors were listed using statistical data. Results: 12 articles made up the final sample of this study, with a higher frequency of publication in 2021 (n: 04). Most of the studies used the retrospective cohort study, with a moderate level of evidence (III). It was found that the main risk factors statistically associated with the occurrence of lower limb amputation in diabetics were: hypertension (n: 06); peripheral neuropathy (n: 06); chronic kidney disease (n: 05); male sex (n: 05); smoking (n: 05); age over 60 years (n: 04); peripheral vascular disease (n: 04); presence of previous amputation, previous ulcer or foot deformities (n: 04); ulcer size (n: 03); diabetic retinopathy (n: 03). Conclusion: It was found that several factors may be involved in the increase in the prevalence of lower limb amputation in diabetic patients, from sociodemographic issues, lifestyle and factors related to the diabetes clinic itself. Prevention and early intervention actions that consider the reduction of these events are necessary. |

Resultados: 12 artículos conformaron la muestra final de este estudio, con mayor frecuencia de publicación en 2021 (n: 04). La mayoría de los estudios utilizaron el estudio de cohorte retrospectivo, con un nivel de evidencia moderado (III). Se encontró que los principales factores de riesgo asociados estadísticamente a la ocurrencia de amputación de miembros inferiores en diabéticos fueron: hipertensión arterial (n: 06); neuropatía periférica (n: 06); enfermedad renal crónica (n: 05); sexo masculino (n: 05); tabaquismo (n: 05); edad mayor de 60 años (n: 04); enfermedad vascular periférica (n: 04); presencia de amputación previa, úlcera previa o deformidades del pie (n: 04); tamaño de la úlcera (n: 03); retinopatía diabética (n: 03). Conclusión: Se encontró que varios factores pueden estar involucrados en el aumento de la prevalencia de amputación de miembros inferiores en pacientes diabéticos, desde cuestiones sociodemográficas, estilo de vida y factores relacionados con la propia clínica de diabetes. Son necesarias acciones de prevención e intervención temprana que contemplen la reducción de estos eventos.Objetivo: Analisar os principais fatores de risco para amputação de membros inferiores em pacientes com pé diabético. Metodologia: Trata-se de uma revisão sistemática realizada por meio do método PRISMA, nas bases de dados eletrônicas PubMed, Scopus e Web of Science, por meio dos seguintes descritores: “limb amputation”, “secondary amputation”, “repeat amputation”, “diabetic foot” e “neuropathy”. Foram incluídos textos completos, disponíveis em português, inglês ou espanhol, publicados entre janeiro de 2016 a dezembro de 2021; estudos que focaram nos fatores associados a amputação de membros inferiores em pacientes com pé diabético; estudos que incluíram pacientes diabéticos acima de 18 anos; estudos cujos fatores foram elencados por meio de dados estatísticos. Resultados: 12 artigos compuseram a amostra final desse estudo, com maior frequência de publicação em 2021 (n: 04). Grande parte das pesquisas utilizaram o estudo de coorte retrospectivo, apresentando nível de evidência moderado (III). Constatou-se que os principais fatores de risco associados estatisticamente a ocorrência de amputação de membros inferiores em diabéticos foram: hipertensão (n: 06); neuropatia periférica (n: 06); doença renal crônica (n: 05); sexo masculino (n: 05); tabagismo (n: 05); idade acima dos 60 anos (n: 04); doença vascular periférica (n: 04); presença de amputação prévia, úlcera anterior ou deformidades nos pés (n: 04); tamanho da úlcera (n: 03); retinopatia diabética (n: 03). Conclusão: Verificou-se que diversos fatores podem estar envolvidos no aumento da prevalência de amputação de membros inferiores em pacientes diabéticos, desde a questões sociodemográficas, estilo de vida e a fatores relacionados a própria clínica do diabetes.
