Resumo: | The objective of this study is to present the latest in terms of good quality scientific evidence on the clinical use of oral antibiotics in the treatment of acne vulgaris, monitoring whether there is proof of their efficacy and safety in patients. This is a narrative literature review, where the electronic databases Scielo, Pubmed and Lilacs were used, with evidence published in the last ten years. Know that systemic based treatment should be considered in the presence of inflammatory acne and those that are difficult to control. In this case, it may involve antibiotic therapy, where the drugs in this line have as a mechanism of action a suppression of P. acnes, increased addition and free circulation fatty acids. However, the prolonged use of antibiotics, regardless of their class, constitutes a risk factor for the appearance of antimicrobial resistance, evidencing the fact that improper prescriptions of oral antibiotics for the treatment of acne must be avoided, and also that in the need to adopt this therapy, it is to consider the most appropriate class for the patient's needs, avoiding misuse. With this, it is important for prescribed professionals to have a broad knowledge of the classes and mechanisms of action of these drugs, in order to promote evidence-based treatment. In addition, it is imperative that new clinical studies be carried out with greater methodological rigor, in order to verify the best choice of acne treatment in its different degrees, guaranteeing the efficacy and safety of the patient. |

Se trata de una revisión narrativa de la literatura, donde se utilizaron las bases de datos electrónicas Scielo, Pubmed y Lilacs, con evidencia publicada en los últimos diez años. Sepa que se debe considerar el tratamiento sistémico en presencia de acné inflamatorio y aquellos que son difíciles de controlar. En este caso, puede implicar una terapia con antibióticos, donde los fármacos de esta línea tienen como mecanismo de acción una supresión de P. acnes, un aumento de la adición y de la libre circulación de ácidos grasos. Sin embargo, el uso prolongado de antibióticos, independientemente de su clase, constituye un factor de riesgo para la aparición de resistencia antimicrobiana, evidenciando que se debe evitar la prescripción inadecuada de antibióticos orales para el tratamiento del acné, así como la necesidad de adoptar En esta terapia, se trata de considerar la clase más adecuada a las necesidades del paciente, evitando su mal uso. Con esto, es importante que los profesionales prescriptores tengan un amplio conocimiento de las clases y mecanismos de acción de estos medicamentos, con el fin de promover tratamientos basados en evidencia. Además, es imperativo que se realicen nuevos estudios clínicos con mayor rigor metodológico, con el fin de verificar la mejor elección del tratamiento del acné en sus diferentes grados, garantizando la eficacia y seguridad del paciente.O objetivo deste estudo é apresentar o que há de mais recente em termos de evidências científicas de boa qualidade sobre o uso clínico dos antibióticos orais no tratamento da acne vulgar, observando se há comprovação de sua eficácia e segurança nos pacientes. Trata-se de uma revisão narrativa de literatura, onde foram utilizadas as bases de dados eletrônicas Scielo, Pubmed e Lilacs, com evidências publicadas nos últimos dez anos. Sabe-se que o tratamento com base sistêmica deve ser considerado na presença de acne inflamatória e naquelas que apresentam difícil controle. Neste caso, ele pode envolver a antibioticoterapia, onde fármacos desta linha têm como mecanismo de ação uma supressão do P. acnes, diminuição da inflamação e dos ácidos graxos de livre circulação. Contudo, a utilização por um tempo prolongado de antibióticos seja qual for a classe, configura fator de risco para o aparecimento de resistência antimicrobiana, evidenciando o fato de que as prescrições indevidas de antibióticos orais para tratamento da acne devem ser evitadas, e ainda, que na necessidade de adoção dessa terapêutica, é importante considerar a classe mais adequada a necessidade do paciente, evitando o uso indevido. Com isso, tem-se a importância de os profissionais prescritores conhecerem amplamente as classes e mecanismos de ação desses medicamentos, para promoverem um tratamento com base em evidências.
