Evaluation of anterograde amnesia after the use of midazolam and valerian-hops in anxious individuals undergoing mandibular third molar extraction in a slipt-mouth, triple-blind, randomized clinical trial

Resumo: This prospective, split-mouth, triple-blind, randomized clinical trial aimed to assess whether there was anterograde amnesia when performing minimal conscious sedation with fixed extract of valerian-hops and midazolam in anxious individuals who underwent tooth extractions impacted lower third molars. The study was completed by 23 individuals aged between 18 and 25 years, of both sexes, with ASA I surgical risk classification and with impacted lower third molars positioned in a similar way bilaterally. The surgical interventions were performed by a single specialist in oral and maxillofacial surgery and traumatology, in two different sessions, with an interval of 21 days between them. Before all surgeries, each participant received an 8 mg dexamethasone capsule. Forty-five minutes before surgery, they randomly received, orally, 1 capsule of Valerian 500 mg – Hops 120 mg or midazolam 15 mg for each side to be operated on. Each participant received 12 acetaminophen 750 mg tablets to take, if needed, postoperatively. A questionnaire was used to analyze anterograde amnesia. Our results showed that midazolam presented better results in relation to anterograde amnesia. We conclude that valerian-hops is not effective in relation to anterograde amnesia, but it manages to establish comfort, few hemodynamic changes and few side effects, being an alternative to be used in the preoperative period of anxious patients who need lower third molar extraction.

Completaron el estudio 23 sujetos con edades comprendidas entre 18 y 25 años, de ambos sexos, con clasificación de riesgo quirúrgico ASA I y con terceros molares inferiores impactados en posición similar bilateral. Las intervenciones quirúrgicas fueron realizadas por un único especialista en cirugía oral y maxilofacial y traumatología, en dos sesiones separadas, con un intervalo de 21 días entre ellas. Antes de todas las cirugías, cada participante recibió una cápsula de 8 mg de dexametasona. Cuarenta y cinco minutos antes de la cirugía, recibieron aleatoriamente por vía oral 1 cápsula de Valeriana 500 mg – Lúpulo 120 mg o midazolam 15 mg por cada lado a operar. Cada participante recibió 12 tabletas de 750 mg de acetaminofén para tomar, si fuera necesario, después de la operación. Se utilizó un cuestionario para analizar la amnesia anterógrada. Nuestros resultados mostraron que el midazolam presentó mejores resultados en relación a la amnesia anterógrada. Concluimos que la valeriana-lúpulo no es efectiva en relación a la amnesia anterógrada, pero logra establecer comodidad, pocos cambios hemodinámicos y pocos efectos secundarios, siendo una alternativa para ser utilizada en el preoperatorio de pacientes ansiosos que necesitan extracción de terceros molares inferiores.Este ensaio clínico, prospectivo, do tipo split-mouth, triplo-cego e randomizado teve como objetivo avaliar se houve amnésia anterógrada ao realizar a sedação consciente mínima com extrato fixo de valeriana-lúpulo e midazolam, em indivíduos ansiosos que foram submetidos a exodontias de terceiros molares inferiores retidos. Concluíram a pesquisa 23 indivíduos com idade entre 18 e 25 anos, de ambos os sexos, com classificação de risco cirúrgico ASA I e com terceiros molares inferiores retidos posicionados de maneira similar bilateralmente. As intervenções cirúrgicas foram realizadas por um único especialista em cirurgia e traumatologia buco-maxilo-facial, em duas sessões diferentes, com um intervalo de 21 dias entre elas. Antes de todas as cirurgias cada participante recebeu uma cápsula de dexametasona 8 mg. Quarenta e cinco minutos antes da cirurgia receberam de forma randomizada, via oral, 1 cápsula de Valeriana 500 mg – Lúpulo 120 mg ou midazolam 15 mg para cada lado a ser operado. Cada participante recebeu 12 comprimidos de paracetamol 750 mg para tomar, se necessário, no pós-operatório. Foi utilizado um questionário para análise da amnésia anterógrada. Nossos resultados mostraram que o midazolam apresentou melhores resultados com relação a amnésia anterógrada.
