Facilitated containment and winding for pain management in preterm infants: randomized crossover clinical trial

Resumo: | This study aimed to analyze the effectiveness of the effects of facilitated restraint and wrapping to reduce pain in premature newborns. A quantitative approach was carried out, in line with the assumptions of an experimental study of the crossover, randomized clinical trial type. Considering the analyzes based on the duration of the painful procedure, changes in heart rate, respiratory rate and oxygen saturation, and the organization according to the subsystems of autonomous, motor and behavioral states of the newborn, it was shown that there was no statistically significant difference between the facilitated restraint and wrapping interventions. However, in the period of application of facilitated restraint, an earlier physiological and behavioral stability was observed, which promoted reorganization, decreased agitation and hemodynamic changes, compared to wrapping. Facilitated containment is performed by positioning the hands, simulating the fetal position. While wrapping consists of keeping the neonate's limbs flexed and the neonate's hands close to the face, an adequate thoracic excursion must be ensured, a position that must be maintained by wrapping the premature's body in tissues, bands or swaddling clothes, which has the function of offering greater safety during the painful procedure. This study revealed efficacy in non-pharmacological interventions, facilitated restraint and wrapping, demonstrating their importance in pain management during procedures that cause low-intensity pain sensitivity in newborns. |

Se realizó un abordaje cuantitativo, en línea con los supuestos de un estudio experimental del tipo ensayo clínico aleatorizado cruzado. Considerando los análisis basados en la duración del procedimiento doloroso, cambios en la frecuencia cardíaca, frecuencia respiratoria y saturación de oxígeno, y la organización según los subsistemas de estados autónomos, motores y conductuales del recién nacido, se demostró que no hubo estadísticamente significativa diferencia entre la sujeción facilitada y las intervenciones de envoltura. Sin embargo, en el período de aplicación de la contención facilitada, se observó una estabilidad fisiológica y conductual más temprana, lo que promovió la reorganización, disminución de la agitación y cambios hemodinámicos, en comparación con el vendaje. La contención facilitada se realiza posicionando las manos, simulando la posición fetal. Si bien el vendaje consiste en mantener las extremidades del neonato flexionadas y las manos del neonato cerca de la cara, se debe asegurar una adecuada excursión torácica, posición que se debe mantener envolviendo el cuerpo del prematuro en pañuelos, bandas o pañales, que tiene la función de ofreciendo mayor seguridad durante el doloroso procedimiento. Este estudio reveló eficacia en intervenciones no farmacológicas, contención facilitada y vendaje, demostrando su importancia en el manejo del dolor durante procedimientos que provocan sensibilidad al dolor de baja intensidad en recién nacidos.Este trabalho teve como analisar a eficácia dos efeitos da contenção facilitada e do enrolamento para redução da dor em recém-nascidos prematuros. Foi realizada uma abordagem quantitativa, em consonância com os pressupostos de um estudo experimental do tipo ensaio clínico randomizado cruzado, crossover. Considerando as análises mediante ao tempo de duração do procedimento doloroso, alterações de frequência cardíaca, frequência respiratória e saturação de oxigênio e, da organização de acordo com os subsistemas de desenvolvimento autônomo, motor e de estados comportamentais do recém-nascido, evidenciou-se que não houve diferença estatisticamente significante entre as intervenções contenção facilitada e enrolamento. No entanto, no período de aplicação da contenção facilitada, observou-se uma estabilidade fisiológica e comportamental mais precoce, que promover a reorganização, diminuiu a agitação e alterações hemodinâmicas, comparado ao enrolamento. A contenção facilitada é realizada pelo posicionamento das mãos, simulando a posição fetal. Enquanto que o enrolamento, consiste em manter os membros em flexão e as mãos do neonato próximas à face, devendo ser garantida uma adequada excursão torácica, posição esta, que deve ser mantida envolvendo o corpo do prematuro em tecidos, faixas ou cueiros, os quais tem a função de oferecer maior segurança durante o procedimento doloroso.
