Clinical, epidemiological and obstetrical aspects of maternal near miss in a reference maternity hospital in Northeast Brazil

Resumo: Maternal near miss can be defined as a woman who almost died, but survived a complication that occurred during pregnancy, delivery, or puerperium, by assistance provided or by chance. The objective of this analytical, documental, and quantitative study was to evaluate the clinical epidemiological and obstetric profile of maternal near miss of patients admitted from May to December 2019 in intensive care beds of a reference maternity hospital in north-eastern Brazil who presented at least one of the diagnostic criteria. Statistical Package for Social Sciences version 25 software was used for statistical analysis. The total number of cases was 77; of these, 36 were associated with eclampsia. The predominant age group was 20 to 25 years; 41.6% were single; 63.6% had completed high school; 85.7% underwent a cesarean section; 81.8% were hospitalized during the puerperium; and 68.8% had no preexisting diseases/associated obstetric conditions. Prior placenta and hypertensive disease were more associated with the cases. Uncontrolled seizure was the most frequent clinical criterion; acute thrombocytopenia, the laboratory one; and hysterectomy, the management one. Thus, it becomes necessary to perform frequent analyses on the obstetric population assisted in a health system and its trajectory in search of care, aiming to assess the quality of care provided, since eclampsia and bleeding complications were the most frequent occurrences among the cases of and these are preventable conditions, with detection of risk factors still in the prenatal period.

El objetivo de este estudio analítico, documental y cuantitativo fue evaluar el perfil epidemiológico y clínico obstétrico de los near miss maternos de las pacientes ingresadas de mayo a diciembre de 2019 en las camas de cuidados intensivos de una maternidad de referencia en el noreste de Brasil que presentaron al menos uno de los criterios diagnósticos. Para el análisis estadístico se utilizó el programa informático Statistical Package for Social Sciences versión 25. El número total de casos fue de 77; de ellos, 36 estaban asociados a la eclampsia. El grupo de edad predominante fue de 20 a 25 años; 41,6% eran solteras; 63,6% habían completado la escuela secundaria; 85,7% fueron sometidas a cesárea; 81,8% fueron hospitalizadas durante el puerperio; y 68,8% no tenían enfermedades preexistentes/condiciones obstétricas asociadas. La placenta previa y la enfermedad hipertensiva estaban más asociadas a los casos. Las convulsiones no controladas fueron el criterio clínico más frecuente; la trombocitopenia aguda, el de laboratorio; y la histerectomía, el de gestión. Por lo tanto, es necesario realizar análisis frecuentes sobre la población obstétrica atendida en un sistema de salud y su recorrido en busca de atención, con el objetivo de evaluar la calidad de la asistencia prestada, dado que la eclampsia y las complicaciones hemorrágicas son las ocurrencias más frecuentes entre los casos de y estas son condiciones evitables, con detección de factores de riesgo aún en el prenatal.O near miss materno pode ser definido como uma mulher que quase morreu, mas sobreviveu a uma complicação ocorrida durante a gravidez, parto ou puerpério, por assistência prestada ou ao acaso. Para a análise estatística foi utilizado o software Statistical Package for Social Sciences versão 25. O total de casos foi de 77; destes, 36 estavam associados à eclâmpsia. A faixa etária predominante foi de 20 a 25 anos; 41,6% eram solteiras; 63,6% possuíam Ensino Médio completo; 85,7% foram submetidas à cesariana; 81,8% estiveram internadas durante o puerpério; e 68,8% delas não possuíam doenças preexistentes/condições obstétricas associadas. Placenta prévia e doença hipertensiva estiveram mais associadas aos casos. A convulsão não controlada foi o critério clínico mais frequente; a trombocitopenia aguda, o laboratorial; e a histerectomia, o de manejo.
