Flexible palatal obturator plate for breastfeeding a baby with cleft lip and palate – case report

Resumo: Cleft lip and palate are congenital malformations originated from the lack of fusion of the maxillary and midnasal processes. Children with these defects have facial anatomical impairment, which can prevent or hinder the performance of important functions, including breastfeeding. In this context, the multidisciplinary surgical treatment of cleft lip and palate is essential for the health of the child with orofacial anomaly and for a good maternal-infant relationship. In newborns, less invasive treatments can efficiently contribute to the adequate nutrition of the child until the correction surgery is performed, such as acetate plates, which act as a resource to occlude the fissure, facilitating suction, breathing, avoiding episodes of regurgitation and favoring orofacial growth. The objective and case report: the objective of this article is to report the case of a baby with cleft lip and palate, who had difficulties in adequate feeding due to the inefficient adaptation of the oral cavity with the maternal breast. Due to the patient's young age, requiring the wait for the surgical procedure, it was chosen to make a flexible palatal obturator plate in order to occlude the cleft of the palate and allow the correct breastfeeding. This apparatus was made, after previous molding of the patient's oral cavity, in the palate region, with EVA (Ethylene Copolymer/Vinyl Acetate), showing good adaptation and correct performance of the desired functions. Conclusion: the flexible palatal obturator plate is an effective and safe option of therapy for breastfeeding children with cleft lip and palate who are not old enough for surgical correction of the orofacial defect.

Los niños con estos defectos tienen deficiencias anatómicas faciales, que pueden impedir o dificultar el desempeño de funciones importantes, incluida la lactancia. En este contexto, el tratamiento quirúrgico multidisciplinario del labio y paladar hendido es fundamental para la salud del niño con anomalía orofacial y para una buena relación materno-infantil. En el recién nacido, tratamientos menos invasivos pueden contribuir a la adecuada nutrición del niño hasta que se realice la cirugía correctora, como las placas de acetato, que actúan ocluyendo la fisura, facilitando la succión, la respiración, previniendo episodios de regurgitación y favoreciendo el crecimiento orofacial. Objetivo y relato de caso: el objetivo de este artículo es reportar el caso de un bebé con labio y paladar hendido, que tuvo dificultad para alimentarse adecuadamente debido a la ineficiente adaptación de la cavidad bucal con la mama. Debido a la corta edad de la paciente, y fue necesario esperar al procedimiento quirúrgico, se decidió realizar una placa obturadora palatina flexible para ocluir el paladar hendido y permitir una correcta lactancia. Este aparato fue realizado, previo moldeado de la cavidad bucal del paciente, en la región del paladar, con EVA (copolímero de Etileno / Acetato de Vinilo), mostrando buena adaptación y correcto desempeño de las funciones deseadas. Conclusión: la placa obturadora palatina flexible es una opción terapéutica eficaz y segura para los niños lactantes con labio leporino y paladar hendido que no tienen la edad suficiente para la corrección quirúrgica del defecto orofacial.Fundamento: Fissuras labiopalatinas são malformações congênitas oriundas da falta de fusão dos processos maxilar e médio-nasal. As crianças portadoras destes defeitos possuem comprometimento anatômico facial, que pode impedir ou dificultar a realização de importantes funções, entre elas, o aleitamento materno. Nesse contexto, o tratamento cirúrgico multidisciplinar da fissura labiopalatina é imprescindível para a saúde da criança portadora da anomalia orofacial e para uma boa relação materno-infantil. Em recém-nascidos, tratamentos menos invasivos podem contribuir para a adequada alimentação da criança até a realização da cirurgia de correção, a exemplo das placas de acetato, que atuam ocluindo a fissura, facilitando a sucção, a respiração, evitando episódios de regurgitação e favorecendo o crescimento orofacial. Objetivo e relato de caso: o objetivo deste artigo é relatar o caso de um bebê com fissura labiopalatina, o qual apresentava dificuldade de alimentação adequada devido à ineficiente adaptação da cavidade oral com o seio materno. Em decorrência da pouca idade do paciente, sendo necessário a espera pelo procedimento cirúrgico, optou-se pela confecção de uma placa obturadora palatina flexível com a finalidade de ocluir a fenda do palato e permitir o correto aleitamento. Esse aparato foi confeccionado, após moldagens prévias da cavidade oral do paciente, em região de palato, com EVA (copolímero de Etileno/Acetato de Vinila), apresentando boa adaptação e correto desempenho das funções almejadas.
