Patellar luxation in consequence of femoral distal rotation in feline: case report

Resumo: The patellar dislocation corresponds to the displacement of the patella of the trochlear groove. In the small animal clinic is one of the most common orthopedic problems, especially in dogs, however the cats are rarely affected. The objective of this study was to report a case of medial patellar dislocation grade IV in feline, through the description of the surgical technique, as well as to evaluate the benefits of the therapy for the welfare of the animal. A feline, by the Brazilian short arrived at the clinic presenting lameness and the patella was moved medially to the physical examination. The X-ray revealed hip dysplasia, bilateral patellar dislocation and rotation in the distal portion of the femur. Surgical therapy was chosen by deepening the trochlear groove with a cylindrical file up to 50% of the patellar thickness and lateral imbrication of the patella with anchorage of the fabela with attachment to the tibial tuberosity at "8" and imbrication sutures in the fibrous joint capsule and on the lateral border of the patellar tendon. The surgical correction provided quality of life to the patient and allowed the animal to resume the activities characteristic of its species.

En la clínica de pequeños animales es uno de los problemas ortopédicos más comunes, especialmente en perros, entretanto los gatos son raramente afectados. El objetivo de este trabajo fue relatar un caso de lesión de la rótula medial de IV grado en felinos, a través de la descripción de la técnica quirúrgica, y también evaluar cuáles son los beneficios y consecuencias terapéuticas para el bien estar animal. Un felino pelo corto brasileño, llegó a la clínica presentando cojera, caminando agachado, y la rótula se desplazaba de forma medial, conforme el examen físico. La radiografía mostró displasia en la articulación de la cadera, lesión de la rótula bilateral y giro en la porción periférica del fémur. Se optó por terapia quirúrgica a través del profundamiento del surco de la tróclea con una lima cilíndrica hasta 50% de la espesura de la rótula y sobreposición lateral de la rótula con fijación del fibrocartílago del tendón del músculo gastrocnemio, con la unión de la tuberosidad de la tibia en forma de "8", y suturas de sobreposición en la cápsula articular fibrosa, y en la borda lateral del tendón de la rótula. La corrección quirúrgica proporcionó calidad de vida al paciente y permitió que el animal pudiera retornar a las actividades características de su especie.A luxação patelar corresponde ao deslocamento da patela do sulco troclear. Na clínica de pequenos animais é um dos problemas ortopédicos mais comuns, especialmente em cães, entretanto os gatos raramente são afetados. O objetivo desse trabalho foi relatar um caso de luxação patelar medial grau IV em felino, através da descrição da técnica cirúrgica, bem como avaliar quais os benefícios da terapêutica para o bem-estar do animal. Um felino, pelo curto brasileiro chegou à clínica apresentando claudicação, andar agachado e patela deslocada medialmente ao exame físico. A radiografia mostrou displasia coxofemoral, luxação patelar bilateral e giro na porção distal do fêmur. Optou-se pela terapia cirúrgica através do aprofundamento do sulco troclear com uma lima cilíndrica até 50% da espessura patelar e imbricação lateral da patela com ancoragem da fabela com ligação à tuberosidade da tíbia em “8” e suturas de imbricação na cápsula articular fibrosa e na borda lateral do tendão patelar.
