Visceral leishmaniasis in southern Brazil: critical analysis of epidemiological evolution

Resumo: Visceral leishmaniasis (VL) was considered non-endemic in the southern region of the country until 2006 when there was the first notification of na autochthonous canine case in the municipality of São Borja in Rio Grande do Sul. The states of Santa Catarina and Paraná the autochthony in canines occurred in the years 2010 and 2012 respectively. In the southern region, dueto its recent installation in addition to the few notifications, there is a lack of inclusion of the disease in differential diagnoses in human and veterinary clinics being the prevalence of VL is greater than the record on notified cases.Thus, the objective was to review the situation of human and canine VL and the presence of vectors in the southern region of the country. Official publications on the disease between 2009 and 2020 were considered and online journal data bases were consulted. Conflicting in formation was found regarding the number of cases and location of the infection, which maybe a consequence of the change in the standardization of human VL records, decentralization and lack of uniformity in CVL records, in addition to the dissemination of diagnoses through out dated annual epidemiological bulletins The sandfly Lutzomyia gaminarai, an endemic species in southern Brazil, requires attention from surveillance and researchers dueto its phylogenetic proximity to Lutzomyia longipalpis.

En la región sur, como fue instalada recientemente, además de las pocas notificaciones, hay falta de inclusión de la enfermedad en los diagnósticos diferenciales en la clínica humana y veterinaria, con prevalencia de LV superior al registro de casos notificados. Así, el objetivo fue revisar la situación de la LV humana y canina (LVC) y la presencia de vectores en la región sur del país. Se consideraron las publicaciones oficiales sobre la enfermedad entre 2009 y 2020 y se consultaron las bases de datos de revistas en línea.  Se encontró información contradictoria en cuanto al número de casos y localización de la infección, lo que puede ser consecuencia del cambio en la estandarización de los registros de LV em humanos, la descentralización y falta de uniformidad en los registros de LVC, además de la difusión de diagnósticos a través de boletines epidemiológicos anuales desactualizados. La mosca de la arena Lutzomyia gaminarai, una especie endémica del sur de Brasil, requiere la atención de la vigilancia y los investigadores, debido a su proximidad filogenética a Lutzomyia longipalpis.A leishmaniose visceral (LV) foi considerada indene na região sul do país até 2006, quando houve a primeira notificação de caso autóctone canino no município de São Borja no Rio Grande do Sul. Nos estados de Santa Catarina e Paraná, a autoctonia em caninos ocorreu nos anos 2010 e 2012 respectivamente. Na região sul, por ser de instalação recente, além das poucas notificações, observa-se a falta da inclusão da enfermidade nos diagnósticos diferenciais na clínica humana e veterinária, sendo a prevalência da LV maior que o registro de casos notificados. Assim, objetivou-se revisar a situação da LV humana e canina (LVC) e a presença dos vetores na região sul do país. Foram consideradas publicações oficiais sobre a doença entre 2009 e 2020 e consultadas as bases de periódicos online. Foram encontradas informações conflitantes quanto ao número de casos e localidade da infecção, que podem ser consequência da mudança na padronização dos registros de LV humana, da descentralização e falta de uniformidade nos registros de LVC, além da divulgação dos diagnósticos através de boletins epidemiológicos anuais desatualizados.
