Prostate cancer: Analysis of survival and prognostic factors by age at diagnosis

Resumo: Purpose: to analyze the probability of survival and prognostic factors after five years of prostate cancer diagnosis in Florianópolis, Santa Catarina. Methods: It is a cohort study with data from the Population-Based Cancer Registry (2008 to 2012) and the Mortality Information System (2008 to 2017) about prostate cancer diagnoses. The survival time was the interval between the date of diagnosis and the date of death or the end of follow-up (60 months). The covariates were age, ethnicity, education, marital status, and extension of disease. Survival analyses, Kaplan Meier analysis, and risk factor analyses, Cox model, were stratified by age group (≤59 years, 60 to 69 years, ≥70 years). Results: 836 men diagnosed with prostate cancer were included and 103 died of cancer during follow-up. The probability of survival was 87,7%. Increasing age reduced survival after the cancer diagnosis, while the metastatic extent of disease was the only independent factor associated with death. Conclusion: Age and advanced stage at the time of cancer diagnosis are key determinants of disease prognosis. It is emphasized the importance of minimizing the time between the onset of symptoms, diagnosis, and start of treatment of prostate cancer.

Métodos: Se trata de un estudio de cohortes con datos del Registro de Cáncer de Base Poblacional (2008-2012), y del Sistema de Información de Mortalidad (2008-2017). El tiempo de supervivencia fue el intervalo entre la fecha del diagnóstico y la muerte o el final del seguimiento. Las variables independientes fueron la edad, el origen étnico, el nivel educativo, el estado civil y la extensión de la enfermedad. Los análisis se estratificaron por grupos de edad. Resultados: Se incluyeron 836 hombres y 103 murieron de cáncer durante el seguimiento. La probabilidad de supervivencia era del 87,7%. El aumento de la edad redujo la supervivencia y la extensión metastásica fue el factor independiente de la muerte. Conclusión: La edad y el estadio avanzado son determinantes para el pronóstico. Es importante reducir el tiempo entre la manifestación de los síntomas y el diagnóstico.Objetivo: Analisar a probabilidade de sobrevida e fatores prognósticos após cinco anos do diagnóstico de câncer de próstata em Florianópolis (SC). Métodos: Trata-se de um estudo de coorte com dados do Registro de Câncer de Base Populacional (2008-2012), e do Sistema de Informação sobre Mortalidade (2008-2017). O tempo de sobrevida foi o intervalo entre a data do diagnóstico e a data do óbito ou final do acompanhamento. As variáveis independentes foram idade, etnia, grau de instrução, estado civil e extensão da doença. As análises foram estratificadas por faixa etária. Resultados: Foram incluídos 836 homens e 103 foram a óbito por câncer durante o acompanhamento. A probabilidade de sobrevida encontrada foi de 87,7%. O aumento da idade reduziu a sobrevida, enquanto a extensão metastática foi o fator independente para o óbito. Conclusão: A idade e o estadiamento avançado são determinantes para o prognóstico do paciente com essa neoplasia.
