Influence of the fractures after falls on the functional abilities of institutionalized and non-institutionalized elderly

Resumo: In view of the increase in the elderly population and the increasing persistence of falls in old age, the purpose of this study was to investigate the influence of fractures caused by falls on the functional abilities of non-institutionalized and institutionalized elderly people. One hundred elderly people of both genders, with a mean age of 76 years and who had a history of fractures due to falls in the upper or lower limbs participated in this research. Among the individuals, 50 (30% male and 70% female) lived in community residences, that is, collective shelters, and 50 (30% male and 70% female) lived in long-term care facilities. Initially, the elderly were recruited through medical records from a public hospital and, after the initial contact, the Functional Independence Measure - FIM scale was applied, asking them about the performance of functional skills before and after the falls. Statistical analysis was performed using the chi-square test (p ≤ 0.05). Significant associations were observed between groups when comparing all functional skills studied before and after falls, however, when comparing data on functional skills after falls between non-institutionalized and institutionalized elderly, no significant association was found. The results suggest that fractures caused by falls interfere with the functional independence of the elderly and that the institutionalization factor does not influence functional recovery in the studied sample.

En esta investigación participaron cien personas mayores de ambos sexos, con una edad media de 76 años y que tenían antecedentes de fracturas por caídas en miembros superiores o inferiores. Entre los individuos, 50 (30% hombres y 70% mujeres) vivían en residencias comunitarias, es decir, albergues colectivos, y 50 (30% hombres y 70% mujeres) vivían en centros de atención a largo plazo. Inicialmente, los ancianos fueron reclutados a través de la historia clínica de un hospital público y, luego del contacto inicial, se aplicó la escala Functional Independence Measure - FIM, preguntándoles sobre el desempeño de habilidades funcionales antes y después de las caídas. El análisis estadístico se realizó mediante la prueba de chi-cuadrado (p ≤ 0.05). Se observaron asociaciones significativas entre los grupos al comparar todas las habilidades funcionales estudiadas antes y después de las caídas, sin embargo, al comparar datos sobre habilidades funcionales después de caídas entre ancianos no institucionalizados e institucionalizados, no se encontró asociación significativa. Los resultados sugieren que las fracturas por caídas interfieren con la independencia funcional del anciano y que el factor de institucionalización no influye en la recuperación funcional en la muestra estudiada.Tendo em vista o aumento da população idosa e a crescente persistência das quedas na terceira idade, o propósito deste estudo foi investigar a influência das fraturas por quedas nas habilidades funcionais de idosos não institucionalizados e institucionalizados. Participaram desta pesquisa 100 idosos, de ambos os gêneros, com idade média de 76 anos e que possuíam histórico de fratura por queda em membros superiores ou inferiores. Dentre os indivíduos, 50 (30% masculino e 70% feminino) moravam em residência comunitária, ou seja, abrigos coletivos e 50 (30% masculino e 70% feminino) em instituições de longa permanência. Inicialmente, os idosos foram recrutados através de prontuários de um hospital público e, depois do contato inicial, foi aplicada a escala para Medida de Independência Funcional - MIF, questionando-os quanto à realização das habilidades funcionais antes e após as quedas. A análise estatística foi realizada por meio do teste qui-quadrado (p ≤ 0,05). Foram observadas associações significativas entre os grupos quando comparadas todas as habilidades funcionais estudadas antes e após as quedas, no entanto, quando comparados os dados das habilidades funcionais após quedas entre os idosos não institucionalizados e institucionalizados não se constatou associação significativa.
