Periorbital emphysema after compressed air trauma: case report

Resumo: Introduction: Orbital emphysema is considered a clinical condition characterized by the presence of air installed inadvertently in the orbital and/or periorbital region. Objectives: To describe a case of trauma to the face caused by compressed air with orbital and temporal emphysema in a pediatric patient. Case report: A 12-year-old male patient was admitted to the Oral and Maxillofacial Surgery and Traumatology Service of the Hospital da Restauração Governador Paulo Guerra – Recife/PE with trauma in the periorbital area after an accident with a compressed air jet. On physical examination, the patient presented slight right eyelid occlusion, edema, crepitation and symptoms on palpation. A computed tomography scan of the face was requested, showing signs suggestive of periorbital emphysema in the right orbit and in the right temporal region, as well as the absence of a fracture in the face. The patient was referred for ophthalmic evaluation, in which subcutaneous emphysema was confirmed in the subconjunctival, periorbital and right temporal region, with no impairment of visual acuity and ocular motility. We opted for conservative treatment with drug prescription and clinical follow-up. The patient remains under outpatient follow-up, with no sign of complications. Conclusions: Considering the absence of fracture and visual alteration, the conservative approach proved to be effective for the case described.

Objetivos: Describir un caso de traumatismo facial por aire comprimido con enfisema orbitario y temporal en un paciente pediátrico. Caso clínico: Paciente varón de 12 años ingresado en el Servicio de Cirugía y Traumatología Oral y Maxilofacial del Hospital da Restauração Governador Paulo Guerra - Recife / PE con traumatismo en la zona periorbitaria tras accidente con chorro de aire comprimido. A la exploración física presenta leve oclusión palpebral derecha, edema, crepitación y sintomatología a la palpación. Se solicita tomografía computarizada de rostro, que muestra un signo sugestivo de enfisema periorbitario en la órbita derecha y en la región temporal derecha, así como la ausencia de fractura en la cara. El paciente fue remitido para valoración oftálmica, en la que se confirmó enfisema subcutáneo en región subconjuntival, periorbitaria y temporal derecha, sin afectación de agudeza visual y motilidad ocular. Se optó por un tratamiento conservador con prescripción de fármacos y seguimiento clínico. El paciente permanece en seguimiento ambulatorio, sin signos de complicaciones. Conclusiones: Considerando la ausencia de fractura y alteración visual, el abordaje conservador demostró ser efectivo para el caso descrito.Introdução: O enfisema orbital é considerado uma condição clínica caracterizada pela presença de ar instalada inadvertidamente na região de órbita e/ou periórbita. Objetivos: Descrever um caso de trauma em face por ar comprimido cursando com enfisema orbital e temporal em paciente pediátrico. Relato de caso: Paciente masculino, 12 anos, deu entrada no serviço de cirurgia e traumatologia Buco-Maxilo-Facial do Hospital da Restauração Governador Paulo Guerra – Recife/PE com trauma em periórbita após acidente com jato de ar comprimido. Ao exame físico o paciente apresentou discreta oclusão palpebral direita, edema, crepitação e sintomatologia à palpação. Foi solicitada uma tomografia computadorizada de face, apresentando sinal sugestivo de enfisema periorbital em órbita direita e em região temporal direita, bem como ausência de fratura em face. Paciente foi encaminhado para avaliação oftálmica, na qual, foi confirmado enfisema subcutâneo em região subconjuntival, periorbital e temporal direita, sem comprometimento de acuidade visual e motilidade ocular. Optou-se por tratamento conservadora com prescrição medicamentosa e acompanhamento clínico. O paciente segue em acompanhamento ambulatorial, sem sinal de complicações.
