Resumo: Pyogenic liver abscess is caused by the development of intrahepatic pus secondary to a local inflammatory reaction due to a bacterial infection. This is a serious condition associated with increased morbidity and mortality in those affected. Therefore, the present case report aims to: describe a severe presentation of liver abscess in the elderly, showing the intercurrences during the hospitalization period and how the patient was managed. As well as reviewing the literature on differences in etiopathogenesis and the bases of diagnosis and treatment. The elderly person in the reported case had abdominal pain, fever, anorexia and weight loss, demonstrating a subacute episode. The therapy was carried out taking into account the probable etiology and combined with complementary laboratory and imaging tests, mainly computed tomography. Antibiotic therapy and image-guided percutaneous drainage proved to be one of the mainstays of treatment

Esta es una condición grave asociada con una mayor morbilidad y mortalidad en los afectados. Por lo tanto, el presente reporte de caso tiene como objetivo: describir una presentación severa de absceso hepático en el anciano, mostrando las intercurrencias durante el período de hospitalización y cómo fue manejado el paciente. Así como revisar la literatura sobre las diferencias en la etiopatogenia y las bases del diagnóstico y tratamiento. El anciano del caso relatado presentaba dolor abdominal, fiebre, anorexia y pérdida de peso, demostrando un episodio subagudo. La terapia se realizó teniendo en cuenta la probable etiología y combinada con pruebas complementarias de laboratorio y de imagen, principalmente tomografía computarizada. La antibioticoterapia y el drenaje percutáneo guiado por imagen resultaron ser uno de los pilares del tratamientoO abscesso hepático piogênico é causado pelo desenvolvimento de pus intra-hepático secundário a uma reação inflamatória local devido a uma infecção bacteriana. Esta é uma condição grave associada ao aumento da morbidade e mortalidade nas pessoas afetadas. Sendo assim, o presente relato de caso tem como objetivo: descrever uma apresentação grave do abscesso hepático no idoso, mostrando as intercorrências durante o período de internação e como o paciente foi manejado. Assim como, rever a literatura sobre as diferenças na etiopatogenia e as bases do diagnóstico e do tratamento. O idoso do caso relatado apresentou dor abdominal, febre, anorexia e perda de peso, demonstrando um episódio subagudo. A terapia foi realizada levando em consideração a provável etiologia e combinada com exames complementares laboratoriais e de imagem, principalmente a tomografia computadorizada.
