Prevalence of multi-resistant bacteria, environmental contamination, and routine practices in a public hospital

Resumo: | The significant increase in antimicrobial-resistant bacteria is a major public health problem. The Intensive Care Units (ICUs), as they have critically ill patients, are the most susceptible and promoters of hospital infections. Acinetobacter baumannii and Klebsiella pneumoniae are important species that cause hospital infections, associated with resistance mechanisms that make the strains more virulent. The purpose of this article was to perform epidemiological tracking of the most prevalent bacterial strains and seek their involvement with hospital infections in a public hospital in Palmas, Tocantins, Brazil. A retrospective study of nosocomial infections from 2017 to 2019 was carried out. In 2019, screening was carried out on bed surfaces, using a sterile swab. In 2020, suspected cases of hospital infection were registered. The samples were sown in specific media and evaluated the resistance profile. Results: a higher prevalence of A. baumannii was found in Intensive Care Units (70.4%) in the respiratory tract and on surfaces. K. pneumoniae showed a significant increase in the ICU and ICU in 2018 and 2019. Professional practices that could contribute to cross-contamination were observed during collections, with little antisepsis and lack of personal protective equipment being identified. The results reinforce the need to create mechanisms for improvement and continuing education for professionals, as well as effective antisepsis processes, and creating outbreak investigation strategies. |

Las Unidades de Cuidados Intensivos (UCI), por haber pacientes críticos, son las más susceptibles y promotoras de infecciones hospitalarias. Acinetobacter baumannii y Klebsiella pneumoniae son especies importantes que causan infecciones nosocomiales, asociadas a mecanismos de resistencia que hacen que las cepas sean más virulentas. El objetivo de este estudio fue realizar un tamizaje epidemiológico de las cepas bacterianas más prevalentes y buscar su implicación con infecciones nosocomiales en un hospital público de Palmas, Tocantins, Brasil. Se llevó a cabo un estudio retrospectivo de infecciones nosocomiales de 2017 a 2019. En 2019, las superficies de las camas se examinaron con un hisopo estéril. En 2020 se registraron casos sospechosos de infección nosocomiales. Las muestras fueron sembradas en medios específicos y evaluadas en el perfil de resistencia. Resultados: se encontró una mayor prevalencia de A. baumannii en las Unidades de Cuidados Intensivos (70,4%) en vías respiratorias y superficies. K. pneumoniae presentó aumento significativo, en UTI y UTI, en 2018 y 2019. Durante las colectas se observaron prácticas de profesionales que podrían contribuir a la contaminación cruzada, con poca antisepsia y falta de equipo de protección personal. Los resultados refuerzan la necesidad de crear mecanismos de mejora y educación continua de los profesionales, así como procesos efectivos de antisepsia y la creación de estrategias de investigación de brotes.O aumento significativo de bactérias resistentes aos antimicrobianos constitui um grande problema de saúde pública. A Acinetobacter baumannii e a Klebsiella pneumoniae são espécies importantes causadoras de infecções hospitalares, associadas a mecanismos de resistências que tornam as cepas mais virulentas. O objetivo deste trabalho foi realizar rastreamento epidemiológico das cepas bacterianas mais prevalentes e buscar seu envolvimento com infecções hospitalares em hospital público de Palmas, Tocantins, Brasil. Foi realizado um estudo retrospectivo de infecções hospitalares de 2017 a 2019. Em 2019 foi realizado um rastreamento em superfícies dos leitos, utilizando swab estéril. Em 2020 foram registrados casos suspeitos de infecção hospitalar. As amostras foram semeadas em meios específicos e avaliadas no perfil de resistência. Resultados: foi encontrado maior prevalência de A. baumannii em Unidades de tratamento Intensivo (70,4%) no trato respiratório e em superfícies. A K. pneumoniae apresentou aumento significativo, na UTI e na UCI, em 2018 e 2019. Durante as coletas foram observadas práticas dos profissionais que poderiam contribuir em contaminações cruzadas, sendo identificada pouca antissepsia e falta de equipamentos de proteção individual.
