Acute kidney injury induced by COVID-19: a literature review

Resumo: Introduction: The COVID-19 infection has presented significant complications to individuals affected by the disease, among them, acute kidney injury has been showing an increasing incidence. Objective: to know the prevalence of kidney injuries associated with COVID-19, as well as its main clinical and laboratory manifestations. Method: A narrative literature review was performed using the Scientific Electronic Library Online (SciELO) and PubMed databases. For the search, the descriptors were used: “Sars-CoV-2”, “COVID-19” and “Acute Kidney Injury”. Complete articles were selected, published in the languages: English, Spanish and Portuguese, during the year 2020 to 2023. 482 scientific productions were pre-selected, of which 59 corresponded to the inclusion criteria: complete articles; in English, Portuguese and Spanish and related to the objective used in this study. Results and Discussion: Reports of previous outbreaks of severe acute respiratory syndrome described the prevalence of acute kidney injury, ranging from 5% to 15% of all cases, requiring the use of renal replacement therapy for 5% to 58% of critically ill patients. The impact of acute kidney injury on the outcomes of patients with SARS (SARS)-CoV-2 remains unclear but is of particular interest for introducing early therapy in acute patients as well as contributing to a better definition of prognosis. The extent (?) to which the development of acute kidney injury is associated with severe COVID-19 is unclear, given its association with relative immune dysregulation and exaggerated inflammatory responses. However, it is concluded that patients with the development of acute kidney injury have a worse prognosis and a higher mortality rate.

Objetivo: conocer la prevalencia de las lesiones renales asociadas a la COVID-19, así como sus principales manifestaciones clínicas y de laboratorio. Método: Se realizó una revisión narrativa de la literatura utilizando las bases de datos Scientific Electronic Library Online (SciELO) y PubMed. Para la búsqueda se utilizaron los descriptores: “Sars-CoV-2”, “COVID-19” y “Acute Kidney Injury”. Se seleccionaron artículos completos, publicados en los idiomas: inglés, español y portugués, durante el año 2020 a 2023. Se preseleccionaron 482 producciones científicas, de las cuales 59 correspondieron a los criterios de inclusión: artículos completos; en inglés, portugués y español y relacionado con el objetivo utilizado en este estudio. Resultados y Discusión: Reportes de brotes previos de síndrome respiratorio agudo severo describieron la prevalencia de daño renal agudo, variando del 5% al 15% de todos los casos, requiriendo el uso de terapia de reemplazo renal para el 5% al 58% de pacientes críticamente enfermos. El impacto de la lesión renal aguda en los resultados de los pacientes con SARS (SARS)-CoV-2 aún no está claro, pero es de particular interés para introducir una terapia temprana en pacientes agudos y contribuir a una mejor definición del pronóstico. La medida (?) en la que el desarrollo de lesión renal aguda se asocia con COVID-19 grave no está clara, dada su asociación con una desregulación inmunitaria relativa y respuestas inflamatorias exageradas. Sin embargo, se concluye que los pacientes con desarrollo de insuficiencia renal aguda tienen un peor pronóstico y una mayor tasa de mortalidad.Introdução: A infecção por COVID-19 tem apresentado complicações significativas aos indivíduos acometidos pela doença, dentre elas, a lesão renal aguda vem apresentando uma incidência cada vez maior. Objetivo: conhecer a prevalência das lesões renais associadas a COVID-19, bem como, suas principais manifestações clínicas e laboratoriais. Método: Foi realizada uma revisão narrativa de literatura, utilizando as bases de dados Scientific Eletronic Library Online (SciELO) e PubMed. Para a busca, foram utilizados os descritores: “Sars-CoV-2”, “COVID-19” e “Injúria Renal Aguda”. Foram selecionados artigos completos, publicados nas línguas: inglês, espanhol e português, durante o ano de 2020 a 2023. Foram pré-selecionadas 482 produções cientificas, das quais 59 correspondiam aos critérios de inclusão: artigos completos; nos idiomas inglês, português e espanhol e relacionados ao objetivo utilizado neste estudo. Resultados e Discussão: Relatórios de surtos anteriores de síndrome respiratória aguda grave descreveram prevalência de lesão renal aguda, variando de 5% a 15% do total de casos, com necessidade de utilização de terapia renal substitutiva para 5% a 58% dos pacientes críticos. O impacto da lesão renal aguda nos resultados dos pacientes com Sars (SARS) -CoV-2 permanecem pouco claros, mas são de particular interesse, para introdução de terapêutica precoce nos pacientes agudos bem como contribuição para uma melhor definição de prognóstico. A medida (?) em que o desenvolvimento de lesão renal aguda está associada à COVID-19 grave, não é clara, dada a sua associação com a desregulação imune relativa e respostas inflamatórias exageradas.
