Surgical treatment of mesiodens in odontopediatric patient: Case report

Resumo: Supernumerary teeth are anomalies in number development observed during routine dental examination. Mesiodens is the dental anomaly most frequently seen in permanent dentition, being considered the most common cause of incisor impaction, followed by odontomas and trauma. The most frequent location is in the anterior region of the maxilla and in most studies, it has a conical shape and a short root, which may be single or in pairs, erupted, included and inverted. The diagnosis and treatment depend on several factors such as: position, type and impaction of the supernumerary tooth next to the adjacent teeth. The present study aims to report the case of a patient, 7 years old, who attended the Dental Clinic of Centro Universitário Tiradentes reporting that the tooth was positioned incorrectly. After clinical and radiographic examination, the presence of a mesiodens found in the region of the left upper central incisor was found. It was also observed the delay in the eruption of the tooth mentioned, therefore, it was decided to perform the surgical removal of the supernumerary. About three weeks after the extraction, the permanent element started to erupt, proving the suspicion that it was being impacted by the supernumerary element. It was then possible to realize that the faster the diagnosis, the better the prognosis, since there may be several consequences such as resorption of the permanent tooth, formation of cysts, malocclusion, aesthetic compromise.

Mesiodens es la anomalía dental visto a menudo en la dentición permanente, siendo considerada la causa más común de impactación de los incisivos, seguida de odontomas y trauma. La localización más frecuente es en la región anterior del maxilar y en la mayoría de los estudios tiene forma cónica y raíz corta, que puede ser única o en pares, erupcionada o incluido en e incluso invertida. El diagnóstico y el tratamiento dependen de varios factores como: posición, tipo e impactación del diente supernumerario junto a los dientes adyacentes. El presente estudio tiene como objetivo reportar el caso de un paciente de 7 años que acudió a la Clínica Dental del Centro Universitario Tiradentes informando que el diente estaba mal colocado. Después del examen clínico y radiográfico se constató la presencia de un mesiodens incluido en la región del incisivo central superior izquierdo. También hubo un retraso en la erupción del mencionado diente, por lo que se decidió realizar la extirpación quirúrgica del supernumerario. Aproximadamente tres semanas después de la extracción, el elemento permanente comenzó a erupcionar, lo que demuestra la sospecha de que estaba siendo impactado por el elemento supernumerario. Entonces fue posible darse cuenta de que cuanto más rápido sea el diagnóstico, mejor es lo pronóstico, ya que puede haber varias secuelas como reabsorción del diente permanente, formación de quistes, maloclusión, compromiso estético. Os dentes supranumerários são anomalias de desenvolvimento de número observados durante o exame clínico e radiográfico de rotina. O mesiodens é a anomalia dentária mais frequentemente observada em dentição permanente, sendo considerada a causa mais comum de impactação dos incisivos, seguida de odontomas e trauma. Acomete a região anterior da maxila e na maioria dos estudos, apresenta-se de forma cônica e raiz curta, podendo ser único ou aos pares, erupcionado ou incluso e até mesmo invertido. O diagnóstico e o tratamento dependem de vários fatores como: posição, tipo e impacção do dente supranumerário junto aos dentes adjacentes. O presente estudo tem como objetivo relatar o caso de um paciente, 7 anos de idade, que compareceu à Clínica Odontológica do Centro Universitário Tiradentes relatando que o dente estava posicionado errado. Após exame clínico e radiográfico constatou-se a presença de um mesiodens incluso na região de incisivo central superior esquerdo. Observou-se também o atraso na erupção do dente supracitado, portanto, optou-se por realizar a remoção cirúrgica do supranumerário. Cerca de três semanas após a extração, o elemento permanente iniciou sua erupção, comprovando a suspeita de que estava sendo impactado pelo elemento supranumerário.
