Replacement of fractured tooth restoration with darkened substrate: Case report

Resumo: Fractured and darkened teeth compromise the aesthetics of the smile and can result from situations of dental trauma, which can also negatively affect the individual's occlusion and psychosocial development. The objective of this study is to report a clinical case of replacement of class IV restoration in composite resin with the use of a silicone palatal guide on a fractured tooth with a darkened substrate. Female patient, 23 years old, sought care complaining of unsatisfactory restoration on tooth 21. It was proposed to replace the restoration and masking of the darkened substrate with direct composite resin. Molding of the upper arch and diagnostic waxing was performed to make a silicone palatal guide. The old restoration was removed; color selection of the restorative material; rubber dam isolation of the operative field; prophylaxis with pumice paste and water; conditioning with 35% phosphoric acid (30 seconds in enamel and 15 seconds in dentin); tooth washing and drying; applications two layers of universal adhesive and light curing. Sequentially, a highly translucent composite resin was inserted into the palatal guide, taken in the mouth, to make the palatal shell; construction of the opaque dentin body; contour definition and characterization with resin for enamel, followed by finishing and polishing. The treatment employed proved to be effective for returning the shape and color compatible with the adjacent teeth. The use of the silicone guide allowed good levels of marginal adaptation and facilitated the step of increasing the restorative material.

El objetivo de este trabajo es reportar un caso clínico de reemplazo de restauración clase IV en resina compuesta con el uso de una guía palatina de silicona sobre un diente fracturado con sustrato oscurecido. Una paciente de 23 años solicitó atención médica quejándose de una restauración insatisfactoria en el diente 21. Se propuso reemplazar la restauración y el enmascaramiento del sustrato oscurecido con resina compuesta directa. Se realizó un moldeado de la arcada superior y un encerado diagnóstico para realizar una guía palatina de silicona. Se quitó la antigua restauración; selección del color del material de restauración; absoluto aislamiento del campo operatorio; profilaxis con pasta pómez y agua; acondicionamiento con ácido fosfórico al 35% (30 segundos en esmalte y 15 segundos en dentina); lavado y secado de dientes; Aplica dos capas de adhesivo universal y fotopolimerizable. Secuencialmente, se insertó una resina compuesta altamente translúcida en la guía palatina, tomada en la boca, para hacer el caparazón palatino; construcción del cuerpo de dentina opaco; Definición y caracterización de contornos con resina para esmalte, seguido de acabado y pulido. El tratamiento empleado demostró ser eficaz para devolver la forma y el color compatibles con los dientes adyacentes. El uso de la guía de silicona permitió buenos niveles de adaptación marginal y facilitó la etapa de estratificación del material restaurador.Dentes fraturados e escurecidos comprometem a estética do sorriso e podem ser decorrentes de situações de traumatismo dentário, o qual também pode afetar negativamente a oclusão e o desenvolvimento psicossocial do indivíduo. O objetivo deste trabalho é relatar um caso clínico de substituição de restauração classe IV em resina composta com uso de guia palatina em silicone em dente fraturado com substrato escurecido. Paciente do gênero feminino, 23 anos buscou atendimento queixando-se de restauração insatisfatória no dente 21. Foi proposta a substituição da restauração e mascaramento do substrato escurecido com resina composta direta. Realizou-se moldagem do arco superior e enceramento diagnóstico para confecção de guia palatina em silicone. Em seguida, procedeu-se com a remoção da restauração antiga; seleção da cor do material restaurador; isolamento absoluto do campo operatório; profilaxia com pasta de pedra pomes e água; condicionamento com ácido fosfórico 35% (30 segundos em esmalte e 15 segundos em dentina); lavagem e secagem do dente; aplicação de duas camadas de adesivo universal e fotopolimerização. Sequencialmente, foi inserida resina composta de alta translucidez na guia palatina, levada em boca, para confecção da concha palatina; construção do corpo opacificado da dentina; definição do contorno e caracterização com resina para esmalte, seguido de acabamento e polimento. O tratamento empregado mostrou-se eficaz para devolução da forma e cor compatíveis com os dentes adjacentes.
