Tratamento cirúrgico de fratura bilateral de mandíbula atrófica em pacientes edêntulos

Resumo: | Atrophic mandible fractures in edentulous patients constitute a subgroup of facial injuries that mostly affect geriatric individuals. Thus, the perioperative protocol of the elderly injured patient must be performed differently from young patients and, many times, they are more complex procedures. Thus, this article aims to review the characteristics, treatment options and complications of surgical treatment of bilateral fracture of the atrophic mandible in edentulous patients. For the construction of this article, a bibliographical survey was carried out in the databases SciVerse Scopus, Scientific Electronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) and ScienceDirect, with the help of the Mendeley reference manager. Articles were covered between the years 2000 to 2023. Surgical treatment is still relatively preferred for atrophic edentulous mandibular fractures based on patient tolerance. Treatment of cases with mild atrophy is similar to that of general mandibular fractures. For cases with severe atrophy, an extraoral incision should be used for fixation and damage to the periosteum should be minimized to preserve the blood supply. The choice of bone plate is most indicated for maximum stability to minimize complications. |

Así, el protocolo perioperatorio del paciente anciano lesionado debe realizarse de manera diferente al de los pacientes jóvenes y, muchas veces, son procedimientos más complejos. Así, este artículo tiene como objetivo revisar las características, opciones de tratamiento y complicaciones del tratamiento quirúrgico de la fractura bilateral de mandíbula atrófica en pacientes edéntulos. Para la construcción de este artículo se realizó un levantamiento bibliográfico en las bases de datos SciVerse Scopus, Scientific Electronic Library Online (Scielo), Estados Unidos. Biblioteca Nacional de Medicina (PUBMED) y ScienceDirect, con la ayuda del gestor de referencias de Mendeley. Los artículos se cubrieron entre los años 2000 y 2023. El tratamiento quirúrgico sigue siendo relativamente preferido para las fracturas mandibulares desdentadas atróficas según la tolerancia del paciente. El tratamiento de los casos con atrofia leve es similar al de las fracturas mandibulares generales. Para casos con atrofia severa, se debe usar una incisión extraoral para la fijación y se debe minimizar el daño al periostio para preservar el suministro de sangre. La elección de la placa ósea es la más indicada para lograr la máxima estabilidad y minimizar las complicaciones.Fraturas de mandíbula atróficas em pacientes edêntulos constituem um subgrupo de lesões faciais que acometem, majoritariamente, indivíduos geriátricos. Dessa forma, o protocolo perioperatório do paciente idoso lesionado deve ser realizado de forma distinta de pacientes jovens e, muitas vezes, são procedimentos mais complexos. Dessa forma, este artigo possui como objetivo revisar as características, opções de tratamento e complicações do tratamento cirúrgico de fratura bilateral de mandíbula atrófica em pacientes edêntulos. Para a construção deste artigo foi realizado um levantamento bibliográfico nas bases de dados SciVerse Scopus, Scientific Eletronic Library Online (Scielo), U.S. National Library of Medicine (PUBMED) e ScienceDirect, com auxílio do gerenciador de referências Mendeley. Os artigos foram contemplados entre os anos de 2000 a 2023. O tratamento cirúrgico ainda é relativamente preferido para fraturas mandibulares edêntulas atróficas com base na tolerância do paciente. O tratamento dos casos com atrofia leve é semelhante ao das fraturas mandibulares gerais. Para os casos com atrofia grave, uma incisão extraoral deve ser usada para fixação e o dano ao periósteo deve ser minimizado para preservar o suprimento sanguíneo.
