Potential serious drug interactions in patients with non-dialytic chronic kidney disease: a worrying frequency

Resumo: | Objective: to evaluate potential severe and moderate drug interactions in patients with chronic kidney disease (CKD) not on dialysis and using polypharmacy. Methodology: cross-sectional study carried out in a city in the center-west of Minas Gerais/Brazil. The population consisted of patients with non-dialysis kidney disease treated at the municipal nephrology clinic. Descriptive analysis was used to characterize the sociodemographic, clinical, and laboratory profile of the population. Bivariate logistic regression analysis was applied to determine factors associated with potential drug interactions. Variables with p-value < 0.20 were taken to the multivariate model. Results: Of the total of 75 participants, most were female, elderly and white. Among them, 60% (45/75) were in stage 3A and 3B of kidney disease. No significant association was found between the use of polypharmacy and Estimated Glomerular Filtration Rate (p <0.671). Among the 630 drug prescriptions evaluated, a total of 833 drug interactions (MI's) were found, with 7.68% (n=64) being severe, 75.27% (n=627) moderate and 17.05% (n= 142) light. Use of polypharmacy was identified in 81.3% (61/75) of patients, with an average of nine medications per patient. Conclusions: high rates of moderate drug interactions were found, in addition to the presence of severe interactions. The association of multiple drugs in pharmacotherapy, especially for elderly patients with kidney disease, is often unavoidable and, therefore, deserves special attention not only from prescribing professionals but also from the entire team in order to safeguard the safety of these patients, avoiding events adverse events related to MIs. |

Metodología: estudio transversal realizado en una ciudad del centro-oeste de Minas Gerais / Brasil. La población estuvo constituida por pacientes con enfermedad renal no diálisis atendidos en la consulta municipal de nefrología. Se utilizó análisis descriptivo para caracterizar el perfil sociodemográfico, clínico y de laboratorio de la población. Se aplicó un análisis de regresión logística bivariada para determinar los factores asociados con posibles interacciones farmacológicas. Las variables con valor de p <0,20 se llevaron al modelo multivariado. Resultados: Del total de 75 participantes, la mayoría eran mujeres, ancianos y blancos. Entre ellos, el 60% (45/75) estaban en estadio 3A y 3B de enfermedad renal. No se encontró asociación significativa entre el uso de polifarmacia y la tasa de filtración glomerular estimada (p <0,671). Entre las 630 prescripciones de medicamentos evaluadas, se encontraron un total de 833 interacciones medicamentosas (IM), siendo el 7,68% (n = 64) graves, el 75,27% (n = 627) moderadas y el 17,05% (n = 142) leves. El uso de polifarmacia se identificó en el 81,3% (61/75) de los pacientes, con un promedio de nueve medicamentos por paciente. Conclusiones: se encontraron altas tasas de interacciones medicamentosas moderadas, además de la presencia de interacciones severas. La asociación de múltiples fármacos en farmacoterapia, especialmente en pacientes ancianos con enfermedad renal, suele ser inevitable y, por tanto, merece una atención especial no solo por parte de los profesionales prescriptores, sino también de todo el equipo con el fin de salvaguardar la seguridad de estos pacientes, evitando eventos adversos relacionados con IM.Objetivo: avaliar as potenciais interações medicamentosas graves e moderadas em pacientes com doença renal crônica (DRC) não dialítica em uso de polifarmácia. Análise descritiva foi utilizada para caracterização do perfil sociodemográfico, clínico e laboratorial da população. Análise de regressão logística bivariada foi aplicada para determinar os fatores associados às potenciais interações medicamentosas. As variáveis que apresentaram valor de p < 0,20 foram conduzidas ao modelo multivariado. Resultados: Do total de 75 participantes a maioria era do sexo feminino, idosa e de cor branca. Entre eles 60% (45/75) se encontravam nos estágios 3A e 3B da doença renal. Não foi encontrado associação significativa entre o uso da polifarmácia e a Taxa de filtração glomerular estimada (p <0,671). Entre as 630 prescrições farmacológicas avaliadas foi encontrado um total de 833 interações medicamentosas (IM´s), sendo 7,68% (n=64) graves, 75,27% (n=627) moderadas e 17,05% (n=142) leves. Foi identificado uso da polifarmácia em 81,3% (61/75) dos pacientes com média de nove medicamentos por paciente. Conclusões: elevadas taxas de interações medicamentosas moderadas foram encontradas, além da presença de interações graves.
