Cultural adaptation and reproducibility of Bristol COPD Knowledge Questionnaire in patients with cronic obstructive pulmonary disease in Brazil

Resumo: In recent years, there has been a growing emphasis on patients and their families' education on their medical conditions. This has led patients to want more information about their conditions, and in part, health professionals have recognized that a certain self-control of the disease is important for patients. The general objective of this study was to adapt to the Brazilian culture and evaluate the reproducibility of the Bristol COPD Knowledge Questionnaire in patients with chronic obstructive pulmonary disease. The present study dealt with an original questionnaire composed of 65 questions was translated into Portuguese by a professional with mastery in The English language. This first version underwent cultural adaptation. Next, the version adapted to eight patients who answered the questionnaire and evaluated the difficulties with the translated words was evaluated. Next, the version adapted to eight patients who answered the questionnaire and evaluated the difficulties with the translated words was evaluated. This second version in Portuguese culturally adapted underwent a retrograde translation into English by another American translator with mastery in both languages. This English version was compared with the original version by the researchers and then by the author of the original questionnaire. The ICC value of 0.83 was found for the overall evaluation of the two applications of the questionnaire. Evaluated separately, six domains presented the ICC above 0.70, four between 0.60 - 0.69 and three below 0.60. The lowest agreement was observed in the domain regarding etiology and the highest, in oral corticosteroids. We conclude that the Bristol Questionnaire adapted to the Portuguese language of Brazil is easy to understand by patients with COPD and proved to be reproducible.

Esto ha llevado a los pacientes a desear más información sobre sus condiciones y, en parte, los profesionales de la salud han reconocido que es importante para los pacientes cierto autocontrol de la enfermedad. El objetivo general es adaptar la cultura brasileña y evaluar la reproducibilidad del Bristol COPD Knowledge Questionnaire en pacientes con enfermedad pulmonar obstructiva crónica. El presente estudio abordó un cuestionario original compuesto por 65 preguntas y fue traducido al portugués por un profesional con conocimiento del idioma inglés. Esta primera versión sufrió una adaptación cultural. Luego, se aplicó la versión adaptada a ocho pacientes que respondieron el cuestionario y se evaluaron las dificultades con las palabras traducidas. Esta segunda versión en portugués culturalmente adaptada se sometió a una traducción inversa al inglés por otro traductor estadounidense con dominio en ambos idiomas. Esta versión en inglés fue comparada con la versión original por los investigadores y luego por el autor del cuestionario original. Se encontró un valor de CCI de 0,83 para la evaluación global de las dos aplicaciones del cuestionario. Evaluados por separado, seis dominios presentaron ICC por encima de 0,70, cuatro entre 0,60 - 0,69 y tres por debajo de 0,60. La concordancia más baja se observó en el dominio relacionado con la etiología y la más alta, en los corticoides orales. Concluimos que el Cuestionario Bristol adaptado al portugués brasileño es fácil de entender por los pacientes con EPOC y demostró ser reproducible.Nos últimos anos, tem havido um crescimento na ênfase da educação de pacientes e de suas famílias sobre as condições médicas deles. Isso tem levado os pacientes a desejarem mais informações sobre suas condições, e, em parte, os profissionais de saúde têm reconhecido que um certo autocontrole da doença é importante para os pacientes. O objetivo geral deste trabalho foi adaptar para a cultura brasileira e avaliar a reprodutibilidade do Bristol COPD Knowledge Questionnaire em pacientes com doença pulmonar obstrutiva crônica. O presente estudo tratou de um questionário original composto de 65 perguntas foi traduzido para a língua portuguesa por um profissional com domínio na língua inglesa. Essa primeira versão passou por adaptação cultural. Em seguida foi aplicada a versão adaptada a oito pacientes que responderam ao questionário e avaliadas as dificuldades com as palavras traduzidas. Essa segunda versão em português adaptada culturalmente passou por uma tradução retrógrada para o inglês, por outro tradutor americano com domínio nas duas línguas. Esta versão em inglês foi comparada com a versão original pelos pesquisadores e depois pelo o autor do questionário original. Encontrou-se o valor de CCI de 0,83 para a avaliação global das duas aplicações do questionário. Avaliados isoladamente seis domínios apresentaram o CCI acima de 0,70, quatro entre 0,60 - 0,69 e três abaixo de 0,60. A menor concordância foi observada no domínio referente à etiologia e a maior, em corticoides orais.
