Endodontic implications in cancer patients irradiated in head and neck: an integrative review

Resumo: | Head and neck cancer is a chronic disease considered a public health problem. Among therapeutic modalities, radiotherapy implies oral complications and structural changes in the dental organ, including pulp modifications. In the presence of pulpoperiapical involvement, the atraumatic endodontics is a treatment of choice since it is more conservative and is able to eliminate the infection, avoiding an extraction with potential risk of osteoradionecrosis. In view of this, the objective of this study was to review the literature on the dental surgeons’ conduct with regard to the endodontic treatment in cancer patients irradiated in head and neck. This is an integrative literature review, with qualitative nature and a descriptive approach, carried out between February and August 2022, in Virtual Health Library portal and in the Scientific Electronic Library Online and PubMed databases, using the search strategy “((endodontics AND radiotherapy AND cancer AND treatment) OR (endodontia AND radioterapia AND câncer AND tratamento))”. All of the nine studies included in this research addressed that every patient irradiated in head and neck area that required endodontic treatment in order to eradicate infections and/or prevent extractions, should be treated following the guiding principles of atraumatic endodontics, and so guaranteeing minimum inflammatory tissue reaction. For this, the dental surgeons should have full control of the atraumatic endodontic treatment, providing a more conservative treatment, excellent prognosis and quality of life to such affected individuals. |

Entre las modalidades terapéuticas, la radioterapia implica complicaciones orales y cambios estructurales en el órgano dentario, entre ellos, modificaciones pulpares. En caso de afectación pulpo-periapical, la endodoncia atraumática es el tratamiento de elección porque es más conservador y consigue eliminar la infección, evitando una extracción con riesgo potencial de osteorradionecrosis. Ante ello, el objetivo de este estudio fue revisar en la literatura el comportamiento del cirujano dentista en relación al tratamiento endodóntico en pacientes oncológicos irradiados en cabeza y cuello. Se trata de una revisión integrativa de la literatura, de carácter cualitativo y enfoque descriptivo, realizada de febrero a agosto de 2022, en el portal Biblioteca Virtual en Salud y en las bases de datos Scientific Electronic Library Online y PubMed, con la estrategia de búsqueda “(( endodoncia Y radioterapia Y cáncer Y tratamiento) O (endodoncia Y radioterapia Y cáncer Y tratamiento))”. Los nueve estudios incluidos en esta investigación abordaron que todos los pacientes irradiados en la región de cabeza y cuello y que necesitaron tratamiento endodóntico para erradicar infecciones y/o prevenir extracciones dentales, deben ser tratados de acuerdo con los principios que guían la endodoncia atraumática y garantizando así una reacción mínima. Para ello, el cirujano dentista debe tener pleno dominio de la técnica de endodoncia atraumática, brindando un tratamiento más conservador, excelente pronóstico y calidad de vida para las personas afectadas.O câncer de cabeça e pescoço é uma doença crônica considerada um problema de saúde pública. Na ocorrência do envolvimento pulpo-periapical, a endodontia atraumática é o tratamento de escolha por ser mais conservadora e por conseguir eliminar a infecção, evitando uma exodontia com potencial risco de osteorradionecrose. Diante disto, o objetivo desse trabalho foi revisar na literatura a conduta do cirurgião-dentista frente ao tratamento endodôntico em pacientes oncológicos irradiados em cabeça e pescoço. Tratou-se de uma revisão integrativa de literatura, com caráter qualitativo e abordagem descritiva, realizada no período de fevereiro a agosto de 2022, no portal da Biblioteca Virtual em Saúde e nas bases de dados Scientific Electronic Library Online e PubMed, com a estratégia de busca “((endodontics AND radiotherapy AND cancer AND treatment) OR (endodontia AND radioterapia AND câncer AND tratamento))”. Os nove estudos incluídos nesta pesquisa abordaram que todos os pacientes irradiados em região de cabeça e pescoço e que necessitavam de tratamento endodôntico para erradicar infecções e/ou prevenir exodontias, deveriam ser tratados obedecendo aos princípios que norteiam a endodontia atraumática e garantindo assim, mínima reação inflamatória nos tecidos.
