New observations on the functions attributed to the insular cortex: an integrative literature review

Resumo: Introduction: The insula is a brain lobe that grows less during development and, therefore, is covered by the frontal, temporal and parietal lobes. Some of its functions are somatosensory processing, visceral information processing, interoception and empathy. Objective: Recognize the structure and functions of the insular cortex, as it is an area of the human cerebral cortex not yet fully understood. Methodology: This is a literature review, carried out in March 2022, in the following databases: National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Library (VHL), Virtual Health Library (VHL) and EBSCO Information Services. Results and Discussion: Current evidence suggests that the insula plays a strong role in conscious feelings by anticipating the body's effects of emotional events. The insular cortex, activated by homeostatic imbalance, or by reward cues, plays a key role in this balancing process, so it is not surprising that the insula is heavily involved in addictive behaviors. In addition, an appropriate level of insular activity is required to provide a warning signal to initiate brain responses to salient stimuli. Conclusion: It is evident that the understanding of the anatomy and functionality of the insular cortex is essential for the elucidation of several neurosensory aspects of the human being, since the insula is functionally heterogeneous, with distinct patterns of connectivity with large-scale brain networks associated with numerous functions and behaviors.

Metodología: Se trata de una revisión bibliográfica, realizada en marzo de 2022, en las siguientes bases de datos: National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Virtual Health Biblioteca (BVS), Biblioteca Virtual en Salud (BVS) y Servicios de Información de EBSCO. Resultados y discusión: La evidencia actual sugiere que la ínsula juega un papel importante en los sentimientos conscientes al anticipar los efectos corporales de los eventos emocionales. La corteza insular, activada por un desequilibrio homeostático o por señales de recompensa, juega un papel clave en este proceso de equilibrio, por lo que no sorprende que la ínsula esté muy involucrada en las conductas adictivas. Además, se requiere un nivel apropiado de actividad insular para proporcionar una señal de advertencia para iniciar respuestas cerebrales a estímulos destacados. Conclusión: Es evidente que la comprensión de la anatomía y funcionalidad de la corteza insular es fundamental para el esclarecimiento de varios aspectos neurosensoriales del ser humano, ya que la ínsula es funcionalmente heterogénea, con distintos patrones de conectividad con redes cerebrales a gran escala asociadas con numerosas funciones y comportamientos.Introdução: A ínsula é um lobo cerebral que cresce menos durante o desenvolvimento e, por isso, é recoberto pelos lobos frontal, temporal e parietal. Algumas de suas funções são processamento somatossensorial, processamento de informações viscerais, interocepção e empatia. Objetivo: Reconhecer a estrutura e as funções do córtex insular, visto que é uma área do córtex cerebral humano ainda não completamente entendida. Metodologia: Trata-se de uma revisão de literatura, realizada no mês de março de 2022, nas seguintes bases de dados: National Library of Medicine (PubMed MEDLINE), Scientific Electronic Library Online (Scielo), Cochrane Database of Systematic Reviews (CDSR), Google Scholar, Biblioteca Virtual em Saúde (BVS), Biblioteca Virtual em Saúde (BVS) e EBSCO Information Services. Resultados e discussão: Evidências atuais sugerem que a ínsula desempenha um forte papel em sentimentos conscientes, ao antecipar o corpo aos efeitos de eventos emocionais. O córtex insular, ativado por desequilíbrio homeostático, ou por dicas de recompensa, tem um papel fundamental neste processo de equilíbrio, por isso, não é surpreendente que a ínsula é fortemente envolvida em comportamentos de dependência. Além disso, um nível apropriado de atividade insular é necessário para fornecer um sinal de alerta para iniciar respostas cerebrais a estímulos salientes.
