Transfer of Health Care from a reference clinic to primary care: paths towards integrality

Resumo: This research was carried out in a pediatric outpatient clinic of a secondary and tertiary health care unit, in the state of Rio de Janeiro. The transfer of care is a set of actions aimed at guaranteeing the access of patients to primary care, the integrality between the levels of care through the cooperation between health care services in partnership with the health care network and with the Primary Care Units (PCUs) at patient’s neighborhood. Promoting and supervising the transfer of children form secondary to primary care becomes the general objective of this research. The results have shown that most transfers were successful and the difficulties reported by some managers are related to the underfunding of the PCUs. The transfers resulted in lower costs for the patients and increased offer of consultations at the outpatient clinic, increasing the access to the adequate level of health care according to the patient’s necessities, respecting SUS principles.

La transferencia de atención es un conjunto de estrategias que tienen como objetivo garantizar el acceso a la atención primaria, con el objetivo de la integralidad entre los niveles de atención, a través de la intersectorialidad en asociación con la red y con las Unidades Básicas de Salud (UBS) del territorio de los usuários. Así, el análisis del proceso de transferencia de atención a los niños en condiciones de cuidado en el (UBS) se convierte en el objetivo general de este artículo. Los resultados indican éxito en la mayoría de las transferencias; y las dificultades reportadas por algunas personas responsables están relacionadas con la infrafinanciación de los UBS. Las transferencias resultaron en un menor costo de viaje para los usuarios y en la mayor oferta de vacantes ambulatorias, ampliando el acceso al nivel de atención de acuerdo con las necesidades de salud, respetando los principios del SUS.Trata-se de resultados da pesquisa de Transferência de Cuidados, realizada em ambulatório de crianças de uma unidade de saúde de média e alta complexidade, que abrange o estado do Rio de Janeiro, para a atenção básica. A transferência de cuidados constitui-se num conjunto de estratégias que visa garantir o acesso à atenção básica, objetivando a integralidade entre os níveis de atenção, através da intersetorialidade em parceria com a rede e com as Unidades Básicas de Saúde (UBSs) do território dos usuários. Desta forma, analisar o processo da transferência de cuidados das crianças em condições de atendimento nas (UBSs) torna-se o objetivo geral deste artigo. Os resultados apontam sucesso na maioria das transferências; e as dificuldades relatadas por alguns responsáveis relacionam-se ao subfinanciamento das UBSs.
