Management of radiodermatitis in cancer patients: integrative review

Resumo: A radiodermatitis directly affects the quality of life of patients undergoing radiotherapy for pain, and local and systemic may occur in some cases. Because they are complex wounds that are difficult to heal, such as cutaneous wounds due to radiodermatitis, they can have repercussions, with social and personal stigma, due to changes in body image, especially in cases of those who radiate regions such as the head, neck and breasts. The present study aimed to verify the scientific evidence and evidence for the management of radiodermatitis in patients with cancer, in national and international literature. This is an integrative review study with five scientific articles selected in the last ten years, using the Virtual Health Library database. Three publications evaluated with a descriptive and cross-sectional design, in which questionnaires were answered about the nurse's performance in relation to the management of radiodermatitis. The need to use evaluation scales for the characterization of radiation-induced toxicity and the development of standards to promote improvements in care practice with standardization of conduct, based on scientific evidence, was highlighted. It was found that this integrative review did not have, among the analyzed publications, any product that started in the topical management of radiodermatitis. The need to carry out further experimental studies such as clinical, randomized, controlled and multicenter trials to support an evidence-based practice is emphasized.

The need to carry out further experimental studies such as clinical, randomized, controlled and multicenter trials to support an evidence-based practice is emphasized.Una radiodermatitis afecta directamente la calidad de vida de los pacientes sometidos a radioterapia para el dolor, y en algunos casos puede ocurrir local y sistémica. Por tratarse de heridas complejas y de difícil cicatrización, como las heridas cutáneas por radiodermatitis, pueden tener repercusiones, con estigma social y personal, por cambios en la imagen corporal, especialmente en los casos de las que irradian regiones como la cabeza, el cuello y las mamas. El presente estudio tuvo como objetivo verificar la evidencia científica y la evidencia para el manejo de la radiodermatitis en pacientes con cáncer, en la literatura nacional e internacional. Se trata de un estudio de revisión integradora con cinco artículos científicos seleccionados en los últimos diez años, utilizando la base de datos de la Biblioteca Virtual en Salud. Tres publicaciones evaluadas con un diseño descriptivo y transversal, en las que se respondieron cuestionarios sobre el desempeño del enfermero en relación al manejo de la radiodermatitis. Se destacó la necesidad de utilizar escalas de evaluación para la caracterización de la toxicidad inducida por radiación y el desarrollo de estándares para promover mejoras en la práctica asistencial con estandarización de conducta, basada en evidencia científica. Se encontró que esta revisión integradora no contaba, entre las publicaciones analizadas, con ningún producto que se iniciara en el manejo tópico de la radiodermatitis. Se enfatiza la necesidad de realizar más estudios experimentales como ensayos clínicos, aleatorizados, controlados y multicéntricos para apoyar una práctica basada en la evidencia.A radiodermite afeta diretamente a qualidade de vida dos pacientes submetidos à terapia radioterápica por serem dolorosas, podendo ocorrer infecções locais e sistêmicas em alguns casos. Por se tratarem de feridas complexas de difícil cicatrização, as lesões cutâneas pela radiodermite podem ter repercussões, com estigma social e pessoal, pelas alterações na imagem corporal, principalmente nos casos de quem irradia regiões como a cabeça, pescoço e mamas. O presente estudo teve como objetivo verificar quais as recomendações e evidências científicas para o manejo da radiodermite em paciente oncológicos, na literatura nacional e internacional. Trata-se de um estudo de revisão integrativa com cinco artigos científicos selecionados nos últimos dez anos, através da base de dados da Biblioteca Virtual em Saúde. Três publicações avaliadas com delineamento descritivo e transversal, nas quais foram respondidos questionários sobre a atuação do profissional enfermeiro frente ao manejo de radiodermite. Destacou-se a necessidade do uso de escalas de avaliação para caracterização da toxicidade induzida pela radiação e o desenvolvimento de protocolos clínicos para promover melhorias na prática assistencial com padronização de condutas, baseada em evidência cientifica. Verificou-se que nesta revisão integrativa não houve dentre as publicações analisadas nenhum produto que demonstrou eficácia no manejo tópico da radiodermite.
