Prevalence and diagnosis of patients with Chronic Lymphocytic Leukemia in the Extreme South of Bahia, Brazil

Resumo: Chronic Lymphocytic Leukemia (CLL) is a proliferative B-cell neoplasm, promoting enlargement of the spleen, liver and lymph nodes, affecting mainly white males, with a high incidence after 65 years of age. Thus, we aimed to identify the prevalence of patients with CLL in the Extreme South of Bahia, analyzing the markers of laboratory diagnostic protocols. The present study is a retrospective cohort from 2015 to 2019, under CAAE Approval: 5.336.684 from CEP/CONEP, of medical records of patients with LLC performed at the High Complexity Oncology Unit - UNACON, located in the municipality of Teixeira de Freitas, Bahia. Thus, the results allowed us to find that, according to information from DATASUS, of the 82 diagnosed cases of chronic lymphoproliferative diseases, between 2015 and 2019 from the Extreme South of Bahia, six cases were of LLC. In the medical records there were clinical-epidemiological characteristics, 66.7% were in male patients; with 50% white and brown individuals, presenting lymphocytosis, thrombocytopenia and normocytic-normochromic anemia (50%), one case (16.7%) carrying Gumprecht's Patches and one case (16.7%) presenting CD79 and IGV15. As for the conclusions, it was seen that the most used diagnostic markers comprised the performance of hematological tests such as hemogram and myelogram, including hematological/biochemical characteristics, especially lymphocytosis, leukocytosis, thrombocytopenia, anemia, with emphasis on immunophenotypic characteristics, by CDs 19 and 79, being the low prevalence in the Far South of Bahia of only 7.3% of cases diagnosed with LLC.

Así, tuvimos como objetivo identificar la prevalencia de pacientes con LLC en el Extremo Sur de Bahía, analizando los marcadores de los protocolos de diagnóstico de laboratorio. El presente estudio es una cohorte retrospectiva de 2015 a 2019, bajo Aprobación CAAE: 5.336.684 del CEP/CONEP, de prontuarios de pacientes con LLC realizados en la Unidad de Oncología de Alta Complejidad - UNACON, ubicada en el municipio de Teixeira de Freitas, Bahía. Así, los resultados permitieron constatar que, según información de DATASUS, de los 82 casos diagnosticados de enfermedades linfoproliferativas crónicas, entre 2015 y 2019 del Extremo Sur de Bahía, seis casos fueron de LLC. En las historias clínicas se encontraron características clínico-epidemiológicas, el 66,7% correspondió a pacientes del sexo masculino; con 50% de individuos blancos y morenos, presentando linfocitosis, trombocitopenia y anemia normocítica-normocrómica (50%), un caso (16,7%) portador de Placas de Gumprecht y un caso (16,7%) presentando CD79 e IGV15. En cuanto a las conclusiones, se observó que los marcadores diagnósticos más utilizados comprendieron la realización de exámenes hematológicos como hemograma y mielograma, incluyendo características hematológicas/bioquímicas, en especial linfocitosis, leucocitosis, trombocitopenia, anemia, con énfasis en características inmunofenotípicas, por CD 19 y 79, siendo la baja prevalencia en el Extremo Sur de Bahía de apenas 7,3% de los casos diagnosticados con LLC.A Leucemia Linfocítica Crônica (LLC) constitui-se como uma neoplasia proliferativa das células B, promovendo aumento do baço, fígado e linfonodos, acometendo principalmente indivíduos do sexo masculino, raça branca e com alta incidência a partir dos 65 anos de idade. O  diagnóstico consiste na realização de exames como hemograma, mielograma, detecção de marcadores celulares agrupadores de diferenciação (CDs). Assim, objetivou-se identificar a prevalência de pacientes com LLC no Extremo Sul da Bahia, analisando os marcadores dos protocolos de diagnóstico laboratoriais. O presente estudo é um coorte retrospectivo de 2015 a 2019, sob Aprovação CAAE: 5.336.684 do CEP/CONEP, de prontuários de pacientes com LLC realizado na Unidade de Alta Complexidade em Oncologia – UNACON, situado no município de Teixeira de Freitas, Bahia. Dessa forma, os resultados permitiram constatar que, conforme informações do DATASUS, dos 82 casos diagnosticados de doenças linfoproliferativas crônicas, entre 2015 a 2019 do Extremo Sul da Bahia, seis casos eram de LLC. Nos prontuários haviam características clínico-epidemiológicas, 66,7% eram em pacientes homens; com 50% de indivíduos brancos e pardos, apresentando linfocitose, plaquetopenia e anemia normocítica-normocrômica (50%), um caso (16,7%) portando Manchas de Gumprecht e um caso (16,7%) apresentando CD79 e IGV15.
