Pseudomeningocele Iatrogênica: Uma revisão de literatura

Resumo: Introduction: Iatrogenic pseudomeningocele is a tear of the dural sac resulting in cerebrospinal fluid (CSF) leakage during spinal surgery such as repair for a herniated disc. It is a cyst covered by fibrous tissue and filled with CSF, being symptomatic or asymptomatic. In this sense, this surgical complication with great harm to the patient, such as post-surgical pain symptoms, should be highlighted. Objective: Narrative review of the published literature on the subject, seeking to gather and synthesize knowledge. Method: literature review with searches in the Virtual Health Library/MEDLINE, CINAHL, Web of Science and Scopus CAPES databases, with the Medical Subject Headings (MeSH) descriptors: “pseudomeningocele”, “adult”, “laminectomy”, associated by the Boolean AND operator, from 2005 to 2021. A total of 92 articles were found, 14 of which were duplicates and 5 were triplicates. Of the 49 articles found, 18 were included, with the aim of answering the guiding question based on the reading in full: “What are the clinical manifestations, diagnoses and treatments used to identify, treat and predict prognosis for patients with pseudomeningocele?”. Results and discussion: cases of pseudomeningocele are asymptomatic, small and spontaneously regress. In symptomatic patients, it is manifested by headache, lumbar pain and radiculopathy. These are risk factors: invasive techniques, use of epidural triamcinolone acetonide, laminectomy and advanced age, foraminectomy, lateral approach and fusion were protective factors. Treatment for asymptomatic patients is conservative; symptomatic, surgical revision for dural repair is recommended. Conclusion: Therefore, its understanding is important to reduce the occurrence and make patients aware of their risk during spinal surgeries.

Trata-se de um cisto revestido por tecido fibroso e preenchido por LCR, sendo sintomático ou assintomático. Nesse sentido, deve-se ressaltar essa complicação cirúrgica com grandes prejuízos ao paciente, como sintomas álgicos no pós-cirúrgico. Objetivo: Revisão narrativa acerca da literatura publicada sobre o assunto buscando reunir e sintetizar o conhecimento. Método: revisão de literatura com pesquisas nas bases de dados Biblioteca Virtual em Saúde/MEDLINE, CINAHL, Web of Science e Scopus CAPES, com os descritores do Medical Subject Headings (MeSH): “pseudomeningocele”, “adult”, “laminectomy'', associados pelo operador booleano AND, no período 2005 a 2021. Foram encontrados 92 artigos, sendo 14 duplicados e 5 triplicados. Dos 49 artigos encontrados, 18 foram incluídos, com o objeto de responder a partir da leitura na íntegra, à pergunta norteadora: “Quais as manifestações clínicas, diagnósticos e tratamentos utilizados para identificar, tratar e prever prognóstico para pacientes com pseudomeningocele?”. Resultados e discussão: os casos de pseudomeningocele é assintomática, pequena e regride espontaneamente. Em sintomáticos, manifesta-se por cefaléia, dor lombar e radiculopatia. Trata-se de fatores de risco: técnicas invasivas, uso de acetonido de triancinolona epidural, laminectomia e idade avançada, foraminectomia, abordagem lateral e fusão foram fatores protetivos. O tratamento para assintomáticos é conservador; sintomáticos, recomenda-se a revisão cirúrgica para reparo dural. Conclusão: Portanto, sua compreensão é importante para reduzir a ocorrência e conscientizar os pacientes sobre seu risco durante cirurgias da coluna vertebral.Introducion: El pseudomeningocele iatrogénico es un desgarro del saco dural que provoca una fuga de líquido cefalorraquídeo (LCR) durante una cirugía de columna, como la reparación de una hernia de disco. Es un quiste cubierto por tejido fibroso y lleno de LCR, siendo sintomático o asintomático. En este sentido, cabe destacar esta complicación quirúrgica con gran perjuicio para el paciente, como son los síntomas de dolor posquirúrgico. Objetivo: Revisión narrativa de la literatura publicada sobre el tema, buscando reunir y sintetizar conocimientos. Método: revisión de la literatura con búsquedas en las bases de datos Biblioteca Virtual en Salud/MEDLINE, CINAHL, Web of Science y Scopus CAPES, con los descriptores Medical Subject Headings (MeSH): “pseudomeningocele”, “adult”, “laminectomy”, asociados por el Operador booleano AND, de 2005 a 2021. Se encontraron un total de 92 artículos, de los cuales 14 fueron duplicados y 5 fueron triplicados. De los 49 artículos encontrados, se incluyeron 18, con el objetivo de responder a la pregunta orientadora a partir de la lectura completa: “¿Cuáles son las manifestaciones clínicas, diagnósticos y tratamientos utilizados para identificar, tratar y predecir el pronóstico de los pacientes con pseudomeningocele?”. Resultados y discusión: los casos de pseudomeningocele son asintomáticos, pequeños y remiten espontáneamente. En pacientes sintomáticos se manifiesta por cefalea, dolor lumbar y radiculopatía. Estos son factores de riesgo: las técnicas invasivas, el uso de acetónido de triamcinolona epidural, la laminectomía y la edad avanzada, la foraminectomía, el abordaje lateral y la artrodesis fueron factores protectores. El tratamiento de los pacientes asintomáticos es conservador; se recomienda la revisión quirúrgica sintomática para la reparación de la duramadre.
