Resumo: Cushing's disease is caused by the corticotrophic pituitary adenoma which increases the level of cortisol and consequently blood glucose levels and the deposition of fat in tissues and organs. This study proposes to report the case of a patient with this rare pathology in order to understand the syndrome that hypercortisolism/adenoma results in and discuss the clinical challenge. This is a case study whose data were obtained by evaluating the patient's medical records, analyzing the diagnostic tests performed and reviewing the literature. The 21-year-old patient complained of difficult-to-control obesity. Physical examination, laboratory and imaging tests were performed, which led to the diagnostic hypothesis of pituitary adenoma. After the surgical removal procedure, the material was sent to the anatomopathological examination for biopsy, which confirmed the diagnosis of corticotrophic pituitary adenoma. It is necessary to pay attention to the clinical abnormalities caused by excess cortisol related to the diagnosis, considering the need for a multidisciplinary treatment.

Este estudio se propone relatar el caso de un paciente con esta rara patología con el fin de comprender el síndrome que provoca el hipercortisolismo/adenoma y discutir el desafío clínico. Se trata de un estudio de caso cuyos datos se obtuvieron mediante la evaluación de la historia clínica del paciente, el análisis de las pruebas diagnósticas realizadas y la revisión de la literatura. El paciente de 21 años se quejaba de obesidad de difícil control. Se realizaron exámenes físicos, de laboratorio y de imagen, lo que llevó a la hipótesis diagnóstica de adenoma hipofisario. Luego del procedimiento de remoción quirúrgica, el material fue enviado al examen anatomopatológico para biopsia, que confirmó el diagnóstico de adenoma hipofisario corticotrófico. Es necesario prestar atención a las alteraciones clínicas provocadas por el exceso de cortisol relacionadas con el diagnóstico, considerando la necesidad de un tratamiento multidisciplinario.Doença de Cushing é causada pelo adenoma de hipófise corticotrófico que aumenta o nível de cortisol e consequentemente os níveis de glicose no sangue e o depósito de gordura nos tecidos e órgãos. Esse estudo propõe relatar o caso de uma paciente portadora dessa patologia rara visando entender a síndrome que o hipercortisolismo/adenoma resulta e discorrer sobre o desafio clínico. Trata-se de um estudo de caso cujos dados foram obtidos por meio de avaliação de prontuário cedido pela paciente, análise dos exames diagnósticos realizados e revisão da literatura. A paciente de 21 anos queixava de obesidade de difícil controle. Foram realizados exame físico, exames laboratoriais e de imagem os quais levaram a hipótese diagnóstica de adenoma de hipófise. Após o procedimento de retirada cirúrgica, o material foi encaminhado ao anatomopatológico para biópsia, que confirmou o diagnóstico de adenoma de hipófise corticotrófico.
