Assessment of maternal morbidity and mortality in HELLP syndrome in a referral maternity hospital in Piauí

Resumo: Introduction: HELLP syndrome in pregnancy is characterized by hemolysis, alteration of liver enzymes and thrombocytopenia, putting the pregnant woman and fetus at risk. Objective: To analyze maternal morbidity and mortality due to HELLP Syndrome in a referral hospital in Piauí. Methodology: The scope was developed through a retrospective analysis of medical records of pregnant and postpartum women from an Intensive Care Unit (ICU) of a public referral maternity hospital in the city of Teresina, Piauí, with a diagnosis of HELLP Syndrome, between January 2016 to December 2018. The study included the quantification of the most used mode of delivery for termination of pregnancy and the number of patients in which terminations of pregnancy occurred, data recorded in Microsoft Excel®. Descriptive statistics were calculated: mean, standard deviation, minimum and maximum, for quantitative variables; and frequencies, for qualitative variables. In the inferential analysis, the Kolmogorov-Smirnov and Student's t tests were used to verify normality and comparison between groups, respectively, in addition, for qualitative independent variables, Pearson's chi-square was performed. Results: From a total of 68, 45.6% (31) of the women had at least one maternal complication. Six (8.8%) maternal deaths were identified. The most prevalent complications were acute respiratory failure 10 (14.7%), acute renal failure 10 (14.7%), infection 10 (14.7%), placental abruption 9 (13.2%), coagulation 7 (10.3%) and 14 (20.6%) presented near miss aspects. Conclusion: The morbidity of pregnant and postpartum women admitted to the ICU of the aforementioned maternity hospital with a diagnosis of HELLP syndrome was high.

Metodología: El trabajo se desarrolló através de un análisis retrospectivo de las historias clínicas de mujeres embarazadas y puérperas de una Unidad de Cuidados Intensivos (UCI) de una maternidad pública de referencia de Teresina, Piauí, con diagnostico de Síndrome HELLP, de enero de 2016 a diciembre de 2018. El estudio incluyó la cuantificación de la modalidad de parto más utilizada para la interrupción del embarazo y el número de pacientes en las que ocurrieron interrupciones del embarazo. Se calcularon estadísticos descriptivos: media, desviación estándar, mínimo y máximo, para variables cuantitativas; y frecuencias, para variables cualitativas. En el análisis inferencial, se utilizaron las pruebas de Kolmogorov-Smirnov y t de Student para verificar la normalidad y comparación entre grupos, respectivamente, además, para las variables independientes cualitativas, se realizó la chi-cuadrado de Pearson. Resultados: De un total de 68, el 45,6% (31) de las mujeres presentaron al menos una complicación materna. Se identificaron seis (8,8%) muertes maternas. Las complicaciones más prevalentes fueron insuficiencia respiratoria aguda 10 (14,7%), insuficiencia renal aguda 10 (14,7%), infección 10 (14,7%), desprendimiento de placenta 9 (13,2%), coagulación 7 (10,3%) y 14 (20,6%). presentó aspectos de Near miss. Conclusión: La morbilidad de las gestantes y puérperas ingresadas en la UCI de la maternidad mencionada con diagnóstico de síndrome HELLP fue alta.Introdução: A Síndrome HELLP na gestação é caracterizada por hemólise, alteração das enzimas hepáticas e plaquetopenia, pondo em risco, gestante e feto. Objetivo: Analisar a morbimortalidade materna por Síndrome HELLP em um hospital de referência do Piauí. Metodologia: O trabalho foi desenvolvido por meio de análise retrospectiva dos prontuários de gestantes e puérperas de uma Unidade de Terapia Intensiva (UTI) de maternidade pública de referência na cidade de Teresina, Piauí, com diagnóstico de Síndrome HELLP, no intervalo de janeiro de 2016 a dezembro de 2018. O estudo incluiu a quantificação da via de parto mais utilizada para interrupção da gestação e o número de pacientes em que ocorreram interrupções da gravidez, dados registrados em Microsoft Excel®. Foram calculadas estatísticas descritivas: média, desvio padrão, mínimo e máximo, para as variáveis quantitativas; e frequências, para as variáveis qualitativas. Na análise inferencial, os testes de Kolmogorov-Smirnov e t de Student foram utilizados para verificação da normalidade e comparação entre os grupos, respectivamente, ademais, para variáveis independentes qualitativas, foi realizado Qui-Quadrado de Pearson. Resultados: De um total de 68, 45,6% (31) das mulheres apresentaram, no mínimo, uma complicação materna. Foram Identificados 6 (8,8%) óbitos maternos. As complicações mais prevalentes foram insuficiência respiratória aguda 10 (14,7%), insuficiência renal aguda 10 (14,7%), infecção 10 (14,7%), descolamento prematuro de placenta 9 (13,2%), distúrbios de coagulação 7 (10,3%) e 14(20,6%) apresentaram aspectos de near miss.
