Prevalence of periodontitis in patients with type 2 diabetes and its association with metabolic control. Cross-sectional study. Cuenca-Ecuador

Resumo: Objective: to determine the prevalence of periodontitis in patients with type 2 diabetes and its association with metabolic control in patients who received medical care in two foundations in the city of Cuenca - Ecuador during the period of January-April 2015. Methodology: cross-sectional, descriptive-analytical study carried out with 95 patients with type 2 diabetes who were evaluated periodontally and in whom the level of metabolic control of diabetes was identified by means of the glycosylated hemoglobin test; in addition, we inquired about sex, age, duration of diabetes, time elapsed since their last medical check-up. Results: 86.32% of patients with periodontitis were identified, the majority of patients with type 2 diabetes had poor metabolic control (87.36%). A statistically significant association was found between the presence of periodontitis and the degree of metabolic control of diabetes (p=0.003), patients with poorly controlled diabetes (HbA1c ≥ 7.0%) presented a higher risk of presenting periodontitis (OR: 6.67; 95% CI: 1.72-26.08); likewise, a statistically significant association was found between the presence of periodontitis and the duration of diabetes (p=0.013), with a higher risk for patients who have had type 2 diabetes for 10 years or more (OR: 6.064; 95% CI: 1.26-29.08). Conclusion: the prevalence of periodontitis in patients with type 2 diabetes was 86.32%; associations were identified both with poor metabolic control of diabetes (HbA1c ≥ 7.0%) and with type 2 diabetes duration greater than 10 years.

Metodología: estudio transversal, descriptivo - analítico desarrollado en 95 pacientes diabéticos tipo 2 que fueron evaluados periodontalmente y en quienes se identificó el nivel de control metabólico de la diabetes, mediante la prueba de hemoglobina glicosilada; además se indagó sobre sexo, edad, tiempo de padecimiento de la diabetes e intervalo de su último control médico. Resultados: se identificó un 86,32% de pacientes con periodontitis, la mayoría de los pacientes diabéticos tipo 2 (87,36%) tenían un control metabólico deficiente. Se encontró una asociación estadísticamente significativa entre la presencia de periodontitis y el grado de control metabólico de la diabetes (p= 0,003), aquellos pacientes mal controlados (HbA1c ≥ 7,0%) presentaron un mayor riesgo de presentar periodontitis (OR: 6,67; IC 95%: 1,72-26,08); así mismo, se determinó una asociación estadísticamente significativa entre la presencia de periodontitis y el tiempo de duración de la diabetes (p= 0,013), con un mayor riesgo para pacientes con 10 años o más de padecimiento de diabetes tipo 2 (OR: 6,064; IC 95%: 1,26-29,08). Conclusión: la prevalencia de periodontitis en diabéticos tipo 2 fue de 86,32%; se identificó su asociación con un mal control metabólico de la diabetes (HbA1c ≥ 7,0%) y con un tiempo de padecimiento de la diabetes tipo 2, mayor a 10 años.    Objetivo: determinar a prevalência de periodontite em diabéticos tipo 2 e sua associação com o controle metabólico, em pacientes atendidos em duas fundações na cidade de Cuenca – Equador, no período de janeiro a abril de 2015. Resultados: 86,32 % dos doentes com periodontite foram identificados, a maioria dos doentes com diabetes tipo 2 tinha um fraco controlo metabólico (87,36 %). Foi encontrada uma associação estatisticamente significativa entre a presença de periodontite e o grau de controle metabólico do diabetes (p=0,003), pacientes mal controlados (HbA1c ≥ 7,0 %) apresentaram um risco aumentado de desenvolver periodontite (OR: 6,67; IC 95 %: 1,72-26,08); também foi encontrada uma associação estatisticamente significativa entre a presença de periodontite e a duração do diabetes (p=0,013), com risco para doentes com 10 anos ou mais de diabetes tipo 2 (OR: 6,064; IC 95 %: 1,26-29,08).
