Barriers to accessing physical rehabilitation after stroke: an integrative review

Resumo: Stroke is one of the main public health problems in the world. It represents the second leading cause of death, and is currently considered the most disabling disease in the world. More than half of the affected patients need rehabilitation, which aims to minimize the sequelae and restore some degree of functional independence, raising their quality of life and reducing the burden generated by the disease. The objective of this review was to know the access to these services offered by the health systems. The search was conducted in the following databases: PubMed, SCOPUS, SciELO, Chocrane, and LILACs. The terms were: "Access to physiotherapy", "stroke", "rehabilitation", and their respective terms in Portuguese present in Medical Subject Headings (MeSH) and in DeCS (Descriptors in Health Sciences). The search included articles published in the last 10 years, in Portuguese, English and Spanish. Of the 353 articles found, 21 met the inclusion criteria for data analysis based on PRISMA guidelines. It was observed in this review that although the approaches and the importance of physical rehabilitation in stroke patients are already well discussed, the access is still difficult, with several barriers such as: insufficient staff, lack of structure, bureaucracy, difficulty in moving around, and lack of understanding about the services. However, the current system of care organization can be improved, using new concepts of structure and care process, creating a more effective communication between the health system, patients and caregivers.

Representa la segunda causa de muerte y actualmente se considera la enfermedad más incapacitante del mundo. Más de la mitad de los pacientes afectados necesitan rehabilitación, cuyo objetivo es minimizar las secuelas y recuperar cierto grado de independencia funcional, aumentando su calidad de vida y reduciendo la carga generada por la enfermedad. El objetivo de esta revisión era conocer el acceso a estos servicios que ofrecen los sistemas sanitarios. La búsqueda se realizó en las siguientes bases de datos: PubMed, SCOPUS, SciELO, Chocrane y LILACs. Los términos fueron: "Access to physiotherapy", "stroke", "rehabilitation", y sus respectivos términos en portugués presentes en el Medical Subject Headings (MeSH) y en el DeCS (Descriptors in Health Sciences). La búsqueda incluyó artículos publicados en los últimos 10 años, en portugués, inglés y español. De los 353 artículos encontrados, 21 de ellos cumplían los criterios de inclusión para realizar el análisis de datos según las directrices PRISMA. Se observó en esta revisión que aunque los enfoques y la importancia de la rehabilitación física en los pacientes con accidentes cerebrovasculares ya están bien discutidos, el acceso sigue siendo difícil, con varias barreras como: personal insuficiente, falta de estructura, burocracia, dificultad de locomoción y falta de comprensión de los servicios. Sin embargo, el sistema actual de organización de la atención puede mejorarse, utilizando nuevos conceptos de estructura y proceso de atención, creando una comunicación más eficaz entre el sistema sanitario, los pacientes y los cuidadores.O Acidente Vascular Cerebral (AVC) é um dos principais problemas de saúde pública no mundo. Representa a segunda principal causa de morte, e é considerada atualmente a doença mais incapacitante do mundo. Mais da metade dos pacientes acometidos necessitam de reabilitação, que objetiva minimizar as sequelas e restabelecer algum grau de independência funcional, elevando sua qualidade de vida e reduzindo a ônus gerado pela doença. O objetivo desta revisão foi conhecer o acesso a esses serviços ofertados pelos sistemas de saúde. A busca foi realizada nas seguintes bases de dados: PubMed, SCOPUS, SciELO, Chocrane e LILACs. Os termos foram: “Access to physiotherapy”, “stroke”, “rehabilitation”, e seus respectivos termos em português presentes no Medical Subject Headings (MeSH) e no DeCS (Descritores em Ciências da Saúde). A busca incluiu artigos publicados nos últimos 10 anos, nos idiomas português, inglês e espanhol. Dos 353 artigos encontrados 21 deles atenderam aos critérios de inclusão para a realização da análise dos dados baseadas nas diretrizes PRISMA. Observou-se nesta revisão que apesar de já bem discutidas as abordagens e a importância da reabilitação física em pacientes acometidos após um AVC, o acesso mostra-se ainda dificultado, com diversas barreiras como: equipe insuficiente, falta de estrutura, burocracia, dificuldade de locomoção, e falta de entendimento sobre os serviços.
