Binge eating disorder: before and after bariatric surgery

Resumo: | Bariatric surgery plays an important role in the treatment of obesity, however, it may be interfered with by psychopathological characteristics such as binge eating, which may be associated with adverse surgical outcomes, interfering with weight loss. Objective: to analyze the clinical and epidemiological profile of patients with binge eating before and after undergoing bariatric surgery, evaluating the influence of this procedure on this pathology. Methodology: cross-sectional, retrospective, survey-type study, carried out between 2020-2021. Sample composed of 5,160 individuals who underwent bariatric surgery, regardless of time, from any region of Brazil. Results: of 5,160 participants, 52.1% did not have and did not develop binge eating, 9.2% had and remained, 36.2% had and improved and 2.3% did not have and developed. There was a predominance: women, median age between 31 and 44 years, southeast region, whites, complete higher education and married. Physical activity was less performed among patients with binge eating, as well as psychological follow-up before surgery. Weight, BMI and time of surgical performance were higher in patients with compulsion and gastric bypass was the most prevalent type. Psychiatric comorbidities were more common among binge patients. Conclusion: binge eating interfered in the results of bariatric surgery, leading to less weight loss or maintenance, obesity permanence and worsening of psychiatric comorbidities. It is necessary to recognize the development and maintenance factors of binge eating to develop strategies to improve surgical outcomes and quality of life in this population. |

Objetivo: analizar el perfil clínico y epidemiológico de los pacientes con atracones antes y después de la cirugía bariátrica, evaluando la influencia de este procedimiento en esta patología. Metodología: estudio transversal, retrospectivo, tipo encuesta, realizado entre 2020-2021. Muestra compuesta por 5.160 personas que se sometieron a cirugía bariátrica, independientemente del tiempo, de cualquier región de Brasil. Resultados: de 5.160 participantes, el 52,1% no tenía y no desarrolló atracones, el 9,2% tuvo y se mantuvo, el 36,2% tuvo y mejoró y el 2,3% no tuvo y desarrolló. Predominó: mujeres, mediana de edad entre 31 y 44 años, región sureste, blancos, educación superior completa y casados. La actividad física se realizó menos entre los pacientes con atracones, así como el seguimiento psicológico antes de la cirugía. El peso, el IMC y el tiempo de realización quirúrgica fueron mayores en los pacientes con compulsión y el bypass gástrico fue el tipo más prevalente. Las comorbilidades psiquiátricas fueron más comunes entre los pacientes con atracones. Conclusión: los atracones interfirieron en los resultados de la cirugía bariátrica, conduciendo a una menor pérdida o mantenimiento de peso, permanencia de la obesidad y empeoramiento de las comorbilidades psiquiátricas. Es necesario reconocer los factores de desarrollo y mantenimiento de los atracones para mejorar los resultados quirúrgicos y la calidad de vida en esta población.A cirurgia bariátrica detém um importante papel no tratamento da obesidade, podendo, no entanto, sofrer interferência de características psicopatológicas como a compulsão alimentar, que pode estar associada a resultados cirúrgicos adversos, interferindo na perda de peso. Objetivo: analisar o perfil clínico e epidemiológico dos pacientes com compulsão alimentar antes e após serem submetidos à cirurgia bariátrica, avaliando a influência desse procedimento nessa patologia. Metodologia: estudo transversal, retrospectivo, tipo survey, realizado entre 2020-2021. Amostra composta por 5.160 indivíduos que realizaram cirurgia bariátrica, independente do tempo, de qualquer região do Brasil. Resultados: dos 5.160 participantes, 52,1% não tinham e não desenvolveram compulsão alimentar, 9,2% tinham e permaneceram, 36,2% tinham e melhoraram e 2,3% não tinham e desenvolveram. Houve predominância: mulheres, idade mediana entre 31 e 44 anos, região sudeste, brancos, ensino superior completo e casados. A atividade física foi menos realizada entre pacientes com compulsão assim como o acompanhamento psicológico antes da cirurgia. O peso, IMC e tempo de realização cirúrgica foram maiores em pacientes com compulsão e o bypass gástrico foi o tipo mais predominante. As comorbidades psiquiátricas foram mais comuns entre pacientes com compulsão. Conclusão: a compulsão alimentar interferiu nos resultados da cirurgia bariátrica, condicionando a uma menor perda ou manutenção do peso, permanência da obesidade e piora das comorbidades psiquiátricas.
